Molecular detection of pathogens in polymicrobial urinary catheter specimens

Abstract:

Infekce močového traktu představují druhé nejčastější infekce vůbec. Přes 40 % nozokomiálních infekcí představuje právě infekce močového traktu, přičemž v 80 % se jedná o infekce spojené s katétrizací (CAUTI). Kromě katétru jsou do močového traktu zaváděny ureterální stenty, oba tyto materiály jsou kolonizovány bakteriemi, které následně mohou vyvolat infekci. Častá polymikrobiální povaha infekcí pak může představovat terapeutický i interpretační problém, protože bakterie aerobní v kombinaci s anaerobními jsou těžko zachytitelné standardní kultivací. V rámci této práce byla navržena a optimalizována molekulárně-biologická metoda záchytu a diagnostiky relevantních bakterií spojených s CAUTI a kolonizátorů stentů. Byla optimalizována PCR-DGGE cílená na V3 a V3–V5 lokusy genu 16S rRNA a pro DGGE sestaveny standardy referenčních bakterií. Optimalizovaná metoda byla využita pro analýzu klinických vzorků moči, sonikátů dolní části stentů, sonikátů horní části stentů. Z celkem 42 vzorků (10 negativních) vykazovalo 21 polymikrobiální charakter (2–5 zástupců). Nejčastěji byli detekováni 2 bakteriální zástupci ve vzorku. Gram negativních bylo detekováno 36 % zástupců (nejčastěji v sonikátech dolní části stentů), gram pozitivních 64 % (nejčastěji v moči). Zástupci byli aerobové ve 20,3 %, fakultativně anaerobní v 56,3 %, a anaerobní v 23,4 %. Nejčastěji zastoupenými rody byly rody Staphylococcus (15,4 %), Corynebacterium (10,8), Acinetobacter a suspektní Escherichia shodně s prevalencí 9,2 %. Bylo prokázáno, že PCR-DGGE je metodický postup, jež je možný využít pro analýzu komplexních vzorků moči a sonikátů stentů.

Abstract:

Urinary tract infections (UTI) represent the second most common infection at all. Over 40 % of nosocomial infections are urinary tract infection, 80 % of UTI are catheterization associated (CAUTI). Ureteral stents are introduced into the urinary tract, apart from the catheters. Both of these materials are colonized by bacteria causing infection. Frequent polymicrobial nature of the infection may then represent a therapeutic and interpretive problem, because culture often fails to detect aerobic bacteria in combination with anaerobes. The aim of this thesis was to designed and optimized culture independent method for detection of bacteria associated with CAUTI and ureteral stents. PCR-DGGE targeting the V3-V5 and V3 16S rRNA gene loci was optimized, standards of reference bacteria was established. The optimized method was used for analysis of the clinical material: urine, bottom stent tip sonicate and upper stent tip sonicate. 21 samples of the 42 (10 negative) were complex (2-5 bacteria). 36 % bacteria were gram-negative (mostly in bottom tip sonicate), 64 % gram-positive (mostly in the urine). Representatives were aerobic in 20.3 % , facultative anaerobic in 56.3%, and anaerobic in 23.4 %. The most frequently represented genera were Staphylococcus (15.4 %), Corynebacterium (10.8 ), Acinetobacter and suspected Escherichia identically with a prevalence 9.2%. It has been demonstrated, PCR-DGGE can be used for analysis of complex samples of urine and stents sonicates.
