Support for parents caring about terminally ill child

Abstract:

Cíl: Cílem práce bylo zjistit potřeby a nenaplněné potřeby rodičů pečujících o nevyléčitelně nemocné dítě z pohledu rodičů i zdravotnických pracovníků. Dále pak byly zkoumány zkušenosti rodičů a jejich spokojenost s poskytovanou podporou dětskými sestrami v péči o nevyléčitelně nemocné dítě. Metodika: Výzkum byl proveden kvalitativní metodou, která pracuje s malým počtem respondentů. Metodou sběru dat probíhal nestandardizovaný rozhovor zaměřený na životní situace rodičů pečující o nevyléčitelně nemocné dítě. Rovněž byl nestandardizovaný rozhovor použit také k šetření se zdravotnickými pracovníky. Empirické šetření probíhalo od měsíce dubna 2011 do září 2011. Výsledky: Rodiče nevyléčitelně nemocných a umírajících dětí odpovídali spontánně na otázky o jejich životních situacích týkajících se okolností souvisejících s onemocněním jejich dítěte. Na základě analýzy rozhovorů byly stanoveny podkategorie. 1. Identifikace problému a onemocnění (podkategorie: sdělení diagnózy, psychický stav rodičů). 2. Životní situace rodiny (podkategorie: omezení či zrušení pracovních závazků, partnerský vztah). 3. Pobyt dítěte v nemocnici (podkategorie: informovanost rodičů o zdravotním stavu, komplikace zdravotního stavu dítěte, psychický stav rodičů při hospitalizaci dítěte, komunikace lékařů a sester. 4. Nabídka pomoci ze strany organizací (podkategorie: potřeby rodičů). 5. Zdravotní stav rodičů. Rozhovorem bylo zjištěno, jakým způsobem podporuje zdravotnický personál rodiče v tomto těžkém období a jakou pomoc si myslí, že by tito rodiče ještě potřebovali. Dále bylo zjištěno, jaké potřeby mají rodiče pečující o tyto děti a zdali došlo k naplnění těchto potřeb. Závěr: Z výpovědí respondentů vyplynulo, že ve většině případů nejsou rodiče dětí spokojeni s přístupem zdravotnického personálu. Onemocnění jejich dítěte byl pro ně velmi traumatizující zážitek a zdravotnický personál jim tuto situaci neulehčoval. Rodiče se domnívali, že zdravotnický personál nemá dostatečné vědomosti o stavu dítěte a o problematice umírání. Rodičům chyběl empatický a lidský přístup. Potřebovali povzbuzení, ovšem to se jim v mnoha případech nedostalo. Rodiče také kladli důraz na finanční stránku po dobu nemoci dítěte. Finanční krizi pociťovali všichni z dotazovaných rodičů. Zdravotní pracovníci se však domnívají, že poskytli rodičům dostatečnou podporu a pomoc.

Abstract:

Object: The purpose of this study is to identify the needs and unfulfilled requirements of parents who are taking care of a child suffering from incurable disease. The study also aims to analyze their experience and contentment with provided services concerning pediatric nursing. Methodology: the study employs quantitative research method including a small number of respondents. The nature of the research consists of interviewing the assisting health care personnel as well as the parents with the intention to learn about their experience on the subject of incurable disease threatening the children. The empirical research took place between April and September, 2011. Results: Parents dealing with a child suffering from an untreatable disease provided spontaneous answers regarding their life experience concerning the circumstances of the child?s condition. Several subcategories were set according to the interviews. 1. Identification of problem and the nature of illness (subcategories: telling the diagnosis, mental state of parents) 2. State of the family (subcategories: changes to employment, the relationship between parents) 3. Child?s hospitalisation (subcategories: knowledge of parents related to the child?s condition, complications adhering to the child?s condition, mental state of parents during child?s hospitalisation, communication between nurses and doctors). 4. Offers by specialised organisation (subcategory: needs of parents) 5. Parents? health status. Interviewing provided sufficient information in order to find out the mechanics behind the routine of medical staff during the progression of child?s disease and what kind of support would be additionally helpful to parents in their opinion. Complementary details gained from each interview supplied also information regarding the fulfilment of parental needs. Conclusion: The vast majority of respondents stated clear parental discontent with the attitude of health care personnel. Tremendous consequences caused by the disease afflicting their children triggered traumatising experience scarcely alleviated by the medical staff. Parents assumed the personnel lack profound knowledge of their child?s condition and thanatology, which suggest the medical approach was devoid of empathy. In this respect, depressed parents felt frequently deprived of necessary encouragement and support despite the contradictory claiming of health care personnel. Beside these inconsistencies with the attitude, all parents accentuated their financial difficulties connected with the disease as well.
