Knowledge and Habits Concerning Selected Neuromonitoring Procedures in Intensive Care among General Nurse

Abstract:

Diplomová práce je zaměřena na problematiku péče o pacienta s kraniocerebrálním poraněním, u něhož je indikováno monitorování centrálního nervového systému. Technický pokrok spolu s lepším chápáním patofyziologických mechanismů traumatického poškození mozku vede k stále většímu využívání monitorování CNS s preferencí multimodálního monitorování, zahrnujícího současné použití více monitorovacích technik, s cílem posoudit perfuzi, oxygenaci a metabolismus mozku. Rozvoj těchto metod klade zvýšené nároky na všeobecné sestry v péči o pacienty s kraniocerebrálním onemocněním. Hlavním cílem diplomové práce je zhodnotit úroveň znalosti a zmapovat zvyklosti v péči o pacienty s kraniocerebrálním onemocněním a v oblasti neuromonitorování u všeobecných sester pracujících na jednotce intenzivní péče a anesteziologicko-resuscitačním oddělení. Na základě analýzy získaných dat jsou vytvořena doporučení pro praxi. Práce se skládá ze dvou částí, teoretické a výzkumné části. Teoretická část se věnuje tématům kraniocerebrálního onemocnění, monitorování CNS a roli sestry v přístrojovém monitorování v rámci ošetřovatelské péče o pacienta s kraniocerebrálním onemocněním. Pozornost je zaměřena zejména na metody monitorování intrakraniálního tlaku, cerebrálního perfuzního tlaku, tkáňové oxymetrie (mozkové oxygenace) a mozkového krevního průtoku. V praktické části práce jsou analyzovány výsledky dotazníkového šetření zaměřeného na zjištění úrovně znalostí a zvyklosti všeobecných sester v péči o pacienta s kraniocerebrálním onemocněním, u něhož je indikováno přístrojové monitorování CNS. Data ve výzkumné části byla vyhodnocena statistickými metodami a slovním popisem.

Abstract:

This thesis deals with the issue concerning the care for patients with craniocerebral diseases in whom central nervous system monitoring is indicated. The technical progress together with a better understanding of pathophysiological mechanisms of traumatic brain damage lead to a more frequent use of CNS monitoring with the preference of multimodality monitoring which comprises a simultaneous use of more monitoring techniques and aims at the assessment of cerebral perfusion, oxygenation and metabolism. The development of these methods is very challenging for general nurses taking care of patients with craniocerebral diseases. The principal objective of this thesis is to determine the level of knowledge and to map the habits in care for craniocerebral disease patients and in neuromonitoring among general nurses working at intensive care units and at departments of anaesthesiology and resuscitation. Several recommendations for practice were made based on the collected data analysis. This thesis consists of two parts – a theoretical one and a research one. The theoretical part features issues relating to craniocerebral diseases, CNS monitoring and the role of a nurse in the apparatus monitoring within the nursing care for a craniocerebral disease patient. Attention is drawn in particular to the monitoring methods of intracranial pressure, cerebral perfusion pressure, tissue oxymetry (cerebral oxygenation) and cerebral blood flow. The practical part analyses the results of the interview survey focusing on the level of knowledge and habits of general nurses in the field of care for craniocerebral disease patients in whom apparatus CNS monitoring is indicated. The data in the research part were evaluated by statistical methods and verbal description.
