How to perform ADL during non-functioning of one hand with use of training and assistive technology

Abstract:

Práce je věnována možnostem provádění denních činností při afunkčnosti jedné ruky s využitím nácviku a kompenzačních pomůcek, a to z pohledu ergoterapeutické intervence. K afunkčnosti horní končetiny může dojít z řady důvodu, významnou skupinou je důsledek prodělané CMP. V teoretické části práce se nachází přístupy a metody, které u pacientů s parézou ci plegií mohou zmírnit disabilitu. Tyto přístupy jsou následně aplikovány v rámci terapie u dvou klientů, jejichž kazuistiky jsou součástí praktické části práce. Cílem práce je poukázat na možnosti při nácviku denních činností, které mohou využít klienti s afunkcností jedné končetiny po prodělání CMP. Terapie se dvěma vybranými klienty, mužem a ženou po ischemické CMP s afunkčností jedné horní končetiny, se zaměřuje na zlepšení poškozených funkcí a využití kompenzačních pomůcek, aby došlo k poklesu míry závislosti v oblastech všedních denních činností, a zlepšila se tak jejich kvalita života. Nejprve došlo k prostudování dokumentace klientů, vstupnímu rozhovoru, ergoterapeutickému vyšetření za užití standardizovaných testů a pozorování při provádění PADL. Na základě těchto dat a momentálních potřeb klientů byl vytvořen ergoterapeutický plán. Intervence zahrnovala u obou klientů především nácvik PADL (přesuny, koupání, oblékání, vertikalizace do stoje), navržení vhodných úprav domácího prostředí a dále navržení a zajištění kompenzačních pomůcek. Využity byly také prvky Bobath konceptu. Po ergoterapeutické intervenci bylo provedeno výstupní vyšetření, které u klientů ukázalo markantní zlepšení ve skoro všech oblastech PADL a rovněž zvýšení Barthel indexu. V praxi se ukazuje, že nácvik různých úkolů v rámci ADL je podstatný pro funkci ruky při její afunkčnosti po prodělané CMP. Jako důležité se rovněž jeví adekvátní úpravy domácího prostředí a vhodné kompenzační pomůcky.

Abstract:

The work is devoted to the possibilities of performing daily activities with the non-functional upper extremity of one hand with the use of training and assistive technology from the point of view of occupational therapy intervention. Dysfunction of the upper extremity can occur for several reasons and it is often a consequence of a stroke. In the theoretical part of the thesis, we analyze approaches and methods that can alleviate disability in patients with paresis or plegia. These approaches are subsequently applied in the framework of therapy with two clients, whose case studies are part of the practical part of the work. The goal of the work is to point out the possibilities for practicing daily activities that can be used by clients with a non-functional upper extremity after being hit by a stroke. Therapy with two selected clients, a man and a woman after an ischemic stroke with a non-functional upper extremity, focuses on improving the damaged functions and the use of assistive technology to decrease the degree of dependence in the areas of daily activities and thus improve their quality of life. First, there was a study of the client's documentation, an initial interview, and an occupational therapy examination using standardized tests and observation during the performance of PADL. Based on this data and the client's current needs, an occupational therapy plan was created. For both clients, the intervention primarily included PADL training (transfers, bathing, dressing, standing up), designing and creating suitable adjustments to the home environment, and designing and creating assistive technology. Elements of the Bobath concept were also used. After the occupational therapy intervention, an outcome examination was performed, which showed an improvement in all areas of PADL and an increase in the value of the Barthel index. The clients experienced a marked improvement in almost all areas of the PADL, as well as an increase in the Barthel index. From praxis, we can see that the training in various tasks within ADL is essential for the function of the upper extremity in its functionality after a stroke. Adequate adjustments to the home environment and the use of assistive technology also appear to be important.
