The subjective reliving the disease in older people with chronic renal failure

Abstract:

Cíl: Cílem práce bylo zjistit, jak senioři subjektivně prožívají život s chronickým selháním ledvin a jeho léčbu. Dílčími cíli pak bylo popsat prožívání v jednotlivých obdobích léčby od prvních příznaků až po současnost. Metodika: Pro dosažení stanoveného cíle byla zvolena kvalitativní výzkumná metoda. Pro sběr subjektivních dat bylo použito deset individuálních polostrukturovaných rozhovorů. Takto získaný materiál byl nejprve doslovně přepsán a dále zpracováván pomocí obsahové analýzy a otevřeného kódování. Získané výsledky vytvořily čtyři oblasti, ke kterým se participanti vyjadřovali. Tyto oblasti byly nazvány: Ambulantní léčba, Dialýza, Vyrovnání se s nemocí a léčbou a Bilancování. Výsledky: Z výsledků výzkumu v této práci vyplývá, že nejproblémovější je smíření se s nově diagnostikovanou nemocí a nutnost výběru dialyzační léčby. Dále se ukázalo, že první dialýza a problémy s cévním přístupem jsou pro participanty spojeny nejen s bolestí, ale také s velkou psychickou zátěží. Dalším poznatkem je, že nutnost dialýzy je krizovou situací, která participanty vrátila zpět do různých fází smiřování se s nemocí. Jako zásadní se v celém procesu jeví pomoc rodinných příslušníků a víra v Boha. Ukázalo se, že participanti neztrácejí naději a smysl života. Překvapující bylo taky zjištění, jak otevřeně hovořili o smrti a umírání. Závěr: Je možné konstatovat, že se většina participantů se svým onemocněním smířila. Svůj život musí do značné míry přizpůsobit obtížím, které jim nemoc přináší. To ale neznamená, že ztratil smysl. Jsou schopni plánovat, své plány realizovat a radovat se z nich.

Abstract:

Aim: The main aim of this thesis is to ascertain how senior citizens subjectively experience their lives with chronic kidney failure and its treatment. The subsidiary aims were to describe the experience with respective periods of the treatment from the first symptoms to the present day. Methodology: Qualitative research method was selected to achieve the designated aim. Ten individual semi-structured interviews were used for subjective data collection. The material thus obtained was first literally transcribed and then further processed using content analysis and open coding. The obtained results formed four areas, on which the participants commented. These areas were designated as: Outpatient Treatment, Dialysis, Acceptance of the Disease and its Treatment, Evaluation. Results: The results of the research in this thesis show that the most difficult issues for the participants are the processes of accepting the newly diagnosed disease and the decision to start the dialysis treatment. The results further showed that the first dialysis session and the problems with vascular access are associated not only with pain but also with a great mental strain for the participant. Another finding is that the necessity for the dialysis creates critical situation, which sends the participants back to various stages of the process of accepting the disease. What proved to be essential in the whole process was help from family members and the participants´ belief in God. The results also showed that the participants do not lose hope and meaning of life. Also surprising was the finding of how openly they spoke about death and dying. Conclusion: It can be concluded that majority of the participants accepted their disease. They must to a large extent adapt their lives to the inconveniences caused by the disease. This does not mean though that their life lost its meaning. They are able to make plans and enjoy their implementation.
