Kinesiotherapy for degenerative changes in the knee joint

Abstract:

Tato bakalářská práce se zabývá využitím kinezioterapie u pacientky s degenerativními změnami v kolenním kloubu bez předchozího operačního řešení. Cílem této práce bylo posoudit vliv dlouhodobé kinezioterapie na zatížení dolní končetiny s degenerativní změnou v kolenním kloubu při chůzi. V teoretické části práce je rozebírána anatomická, kineziologická a biomechanická problematika, potřebná pro lepší pochopení vzniku a řetězení patologií nejen dolní končetinou. Z degenerativních změn kolenního kloubu jsou popsány změny menisků, patelofemorálního skloubení a největší část je věnována gonartróze. U všech postižení jsou nabízené možnosti jejich řešení. Dále je v této části rozebrána chůze, její jednotlivé části, změny jejího stereotypu u osob s gonartrózou a možnosti zhodnocení. V praktické části práce je prezentována kazuistika obsahující anamnézu, vstupní vyšetření, cíl, plány i souhrnný popis průběhu terapií a výstupní vyšetření. Součástí jsou také popisy jednotlivých cviků zařazovaných do terapií. Předmětem terapií byla stimulace chodidla, aktivace nožní klenby s čtyřbodovou oporou, centrace kloubů dolních končetin, aktivace nitrobřišního tlaku, nácvik zatížení chodidla v průběhu stojné fáze kroku s aktivním odvalem palce a samotný nácvik vědomé chůze. Objektivizace terapie byla provedena pomocí kineziologického rozboru a systémem zebris. Klinicky došlo ke snížení bolesti kolenních kloubů, zlepšení centrace kloubů dolních končetin a zatížení chodidel. Při chůzi byla prodloužena stojná fáze pravé dolní končetiny, prodloužila se délka kroků a odraz se stal dynamičtějším. Celkově se stala chůze více koordinovaná a symetrická.

Abstract:

This bachelor thesis deals with the use of kinesiotherapy in a patient with degenerative changes in the knee joint without previous surgery. The study evaluates the impact of long-term kinesiotherapy on the biomechanical loading of a lower limb with degenerative changes in the knee joint during walking. The theoretical part of the thesis discusses the anatomical, kinesiological and biomechanical issues necessary for a better understanding of the origin and chaining of pathologies not only in the lower limb. Among the degenerative changes of the knee joint, changes of the menisci, patellofemoral articulation are described and the largest part is devoted to gonarthrosis. Options for management are offered for all the disabilities. In addition, this section discusses gait, its different parts, changes in its stereotype in individuals with gonarthrosis and the possibilities of evaluation. In the practical part of the thesis, a case study is presented, including medical history, initial examination, the goal, plans and a summary description of the course of therapy and exit examination. Descriptions of the different exercises included in the therapies are also included. The therapies included foot stimulation, activation of the plantar arch with four-point support, centering of the lower extremity joints, activation of intra-abdominal pressure, training of foot loading during the standing phase of the stride with active toe roll, and actual training of conscious gait. Objectification of the therapy was performed using kinesiological analysis and the zebris system. Clinically, there was a reduction in knee joint pain, improvement in lower limb joint centration and foot loading. During gait, the standing phase of the right lower limb was prolonged, the stride length was increased, and the rebound became more dynamic. Overall, gait became more coordinated and symmetrical.
