Prostate Cancer

Abstract:

Karcinom prostaty je zhoubným nádorem a v rozvinutých zemích je nejběžněji diagnostikovaným nekožním zhoubným nádorem u mužů. V ČR je karcinom prostaty třetí nejčastější formou nádorového onemocnění, a to po karcinomu plic a tlustého střeva. Postihuje muže většinou až po padesátém roce života. Na vznik karcinomu prostaty má vliv věk, prostředí, rasové, ale také dietetické faktory. Velký důraz je také kladen na genetické predispozice, kdy je zvýšené riziko u mužů, v jejichž rodině se již karcinom vyskytl u dvou a více členů. Je také prokázáno, že s progresí karcinomu je spojeno mnoho chromosomových a genetických abnormalit. Cílem mé bakalářské práce je studie karcinomu prostaty, možnosti současné moderní diagnostiky karcinomu prostaty včetně genetických a prognostických faktorů na molekulární úrovni. Teoretická část je věnována kromě samotného onemocnění, jeho diagnostice a terapii, také anatomii, histologii prostaty, cytologickému zpracování biologického materiálu. Další část byla zaměřena na epidemiologii jak ve světě, tak u nás v ČR a jiná onemocnění, jež mohou prostatu postihnout. Praktická část se zabývá především metodikou průkazu karcinomu prostaty a Gleasonovým gradingovým systémem, jež je významným prediktorem průběhu onemocnění. Jeho přesné stanovení je důležitým výchozím bodem jak pro urology, tak i onkology. Určitá míra diskrepance mezi Gleasonovým skóre v bioptických vzorcích a v preparátech po radikální prostatektomii je všeobecně známá. Cílem praktické části byla analýza těchto rozdílů v běžné diagnostické praxi rutinního pracoviště patologie a srovnání získaných výsledků s nejnovější dostupnou literaturou.

Abstract:

Prostate cancer is a malignant tumor and in developed countries the most commonly diagnosed non-skin cancer among men. In the Czech Republic, prostate cancer is the third most frequent form of tumor disease, after lung cancer and colon cancer. It affects men mostly after they reach their fifties. Age, environment, racial but also dietary factors have an impact upon the beginning of prostate cancer . Great emphasis is placed on genetic predisposition, where there is an increased risk for men coming from family in which has cancer occurred in two or more members. It has been proven that the progression of cancer involves many genetic and chromosomal abnormalities too. The aim of my thesis is a study of prostate cancer, the current modern diagnostic options for prostate cancer, including genetic and prognostic factors at the molecular level. The theoretical part is devoted to, besides the disease, its diagnosis and therapy, as well as anatomy, prostate histology, cytological processing of biological material. Next part is focused on the epidemiology of both the world and our republic, and other diseases that can affect the prostate. The practical part deals mainly with the provableness of the prostate cancer methodology and Gleason grading system, which is important predictor of the disease progression. Its precise determination is the important starting point for both urologists and oncologists. Certain amount of discrepancies between the Gleason score in biopsy specimens and preparations after radical prostatectomy is generally known. The aim of the practical part was the analysis of these differences in common diagnostic practise of routine pathology department and the comparison of gained results with the latest available literature.
