Economic analysis of a selected disease from the perspective of a health care provider

Abstract:

Úvod: Práce je zaměřena na analýzu ekonomických parametrů léčby vrozené vývojové vady pes equinovarus congenitus pro poskytovatele zdravotních služeb, dále pak na zjištění finanční a časové náročnosti léčby vady pro rodiče léčených dětí. Hlavní cíl: Zjistit náklady a výnosy spojené s léčbou PEC pro poskytovatele zdravotních služeb. Metodika: První část ekonomické analýzy byla vypracována na základě dat poskytnutých ekonomickým úsekem FN Bulovka a je zpracována do tabulek v empirické části práce. Data pochází z let 2015-2021 a zohledňují oba typy léčby, jak konzervativní, tak operační. Druhá část práce byla zpracována pomocí kvantitativního nestandardizovaného a anonymního dotazníkového šetření vlastní konstrukce. Výsledky: Na základě analýzy z ekonomických dat podle předpokladu vyplývá, že finančně nákladnější a náročnější je pro poskytovatele zdravotní péče léčba operační. Konzervativní léčba je náročnější na časový harmonogram rodičů a jejich psychickou zátěž. Na základě analýzy dat z dotazníků je zřejmé, že vada není pro rodiče natolik finančně náročná, jak bylo myšleno v prvopočátku. Připisuji tento závěr tomu, že velké procento léčby hradí malým pacientům zdravotní pojišťovna v rámci veřejného zdravotního pojištění. Z odpovědí rodičů vyplývá také fakt, že někteří musí za specialisty dojíždět mnoho kilometrů a pro tuto skupinu pacientů je léčba časově i finančně náročnější. Závěr: Z tabulek modelových pacientů je zřejmé, že konzervativní i operační léčba pacientů s pes equinovarus congenitus je poměrně nákladná. Přestože náklady na Ponsetiho léčbu jsou překvapivě vyšší, než jsme očekávali, stále je tato léčba ekonomicky nejvýhodnější. Nezanedbatelným faktem je i to, že medicínsky je tato léčba i nejefektivnější. Na základě dotazníkového šetření je zřejmé, že léčba vady pes equinovarus congenitus není tak finančně náročná, jak bylo předpokládáno na začátku, před sestavením dotazníku. Abstract:

Introduction: The work is focused on the analysis of economic parameters of the treatment of congenital malformations of the dog equinovarus congenitus for health care providers, as well as on the financial and time requirements of the treatment of the defect for parents of treated children. Main goal: To determine the costs and benefits associated with PEC treatment for health care providers. Methodology: The first part of the economic analysis was processed on the basis of data provided by the economic department of the Bulovka University Hospital and is processed into tables in the empirical part of the work. The data come from the years 2015-2021 and take into account both types of treatment, both conservative and surgical. The second part of the work was processed using a quantitative non-standardized and anonymous questionnaire survey of their own design. Results: Based on the analysis, economic data show that surgical treatment is more costly and demanding for healthcare providers. Conservative treatment is more demanding on the time schedule of parents and their psychological burden. Based on the analysis of the data from the questionnaire, it is clear that the defect is not as costly for the parents as was initially thought. I attribute this conclusion to the fact that a large percentage of treatment is reimbursed to small patients by the health insurance company under public health insurance. Parents' answers also show that some have to travel many kilometers for specialists, and for this group of patients, treatment is more time-consuming and costly. Conclusion: From the tables of model patients it is clear that conservative and surgical treatment of patients with equinovarus congenitus is relatively expensive. Although the cost of Ponseti treatment is surprisingly higher than we expected, this treatment is still the most economically advantageous. An important fact is that this treatment is the most effective medically. Based on the questionnaire survey, it is clear that the treatment of a defect in the dog equinovarus congenitus is not as expensive as expected at the beginning, before compiling the questionnaire.
