Abstract:

První sezení v psychoterapii je první terapeutickou zkušeností klienta a může být v potenciálním pokračování terapie rozhodující. Pokud je setkání úspěšné, může klient už po samotném prvním sezení dosáhnout žádané změny, nebo je navázána terapeutická spolupráce mezi jím a terapeutem. Toto téma je v odborné literatuře často probíráno, často však bohužel s nízkou výzkumnou podporou. Ve výzkumu samotném je téma prozkoumáno spíše fragmentárně. Dosud chybí (nebo jsem alespoň nenalezla) studii, která by odpověděla souhrnně na to, co je pro klienty v prvním sezení psychoterapie nápomocné. S cílem prozkoumat tuto otázku byl navržen kvalitativní výzkum, který v rozhovorech s osmi respondenty mapoval, co pro ně bylo užitečné celkem v jedenácti prvních sezeních psychoterapie. Ke sběru dat i analýze byla zvolena metoda Zakotvené teorie. Výsledky poukazují na důležitost pocitů, které má klient během a po sezení – pocit „Sedí mi to“, stejně jako Bezpečí a Naděje jsou hlavním měřítkem užitečnosti různých aspektů sezení a dále také výrazně ovlivňují celý proces. Byly odkryty Základní principy, které by v prvním sezení měly být naplněny – Zaměření terapeuta na klienta, atributy (Téměř) přátelského vztahu a Transparentnost. Na proces i klientův dojem působí Intervenující vlivy: Prostředí, ve kterém se setkání odehrává je pro klienty důležité, stejně jako naplnění Potřeb a očekávání, se kterými do terapie vstupují, a na celé sezení pak mají vliv i různé Okolnosti, které obvykle nelze ovlivnit, avšak mohou výrazně přispět k užitečnosti celého sezení pro klienta. Přínos i limity vytvořeného konceptu jsou zváženy v závěru práce.

Abstract:

The first psychotherapeutic session is the first client's therapeutic experience and may be critical in the question of potential continuation of therapy. If the meeting is successful, the client may achieve the desired changes after this very first session, or therapeutic alliance between him and the therapist is established. This topic appears in the psychotherapeutic literature very often, but unfortunately usually with low research support. In the research itself this issue is explored rather fragmentary. So far I have not found any research study that would respond to “what is helpful for clients in the first session of psychotherapy?” Qualitative research was designed in order to explore this question. Interviews with eight respondents have explored what was helpful for them in first psychotherapeutic sessions (11 sessions in total). Grounded Theory method was chosen for collecting data and analysis. The results highlight the importance of clients' feelings during and after sessions – feelings "Fits me well", as well as the Safety and Hope represents the main measure of usefulness of various session aspects, and also strongly influence the process. Research has uncovered Fundamental principles of first session – Client centered approach, (Almost) friendly relationship and Transparency. Intervening influences affect client's perception and whole process. Environment (in which the meeting takes place) is important for clients, as well as fulfilling the Needs and expectations (with which the client enters a therapy). The whole session is also affected by various Circumstances that usually cannot be influenced, but they can significantly contribute to the usefulness of the session for the client. Potential contribution and limits of in here created concept are considered in the conclusion.
