Acute pyelonephritis in children- magnetic resonance imaging as a new promising tool compared with static renal scintigraphy

Abstract:

Statická scintigrafie ledvin s aplikací radiofarmaka 99mTc-DMSA je zlatým standardem k prokázání zánětlivých změn v parenchymu ledvin při akutní pyelonefritidě a následných jizev v renálním parenchymu s časovým odstupem po akutním zánětu ledvin. DWI-MR (Diffusion-weighted magnetic resonance imaging) - magnetická rezonance s difúzně váženými obrazy je novou a citlivější metodou mj. k detekci zánětlivých ložisek, včetně ledvin. Cílem studie byla detekce akutních zánětlivých změn v parenchymu ledvin u dětí s akutní pyelonefritidou pomocí nové metody DWI-MR a srovnání získaných výsledků, a tedy citlivosti této metody, s nálezy dosud standardně používané statické scintigrafie ledvin. Věková hranice nad 3 roky byla zvolena vzhledem k snazšímu vyšetřování těchto dětí bez nutné celkové anestézie nebo sedace. Sekundárním cílem práce bylo zjistit, zda lze tuto metodu uplatnit i v detekci chronických změn v parenchymu ledvin po šesti měsících a stanovit rozsah a charakter chronických změn a jejich anatomický vztah k akutním lézím a výsledky porovnat s výsledky DMSA.

Abstract:

Static 99mTc-DMSA renal scintigraphy is the gold standard for detection of acute inflammatory changes in the renal parenchyma in patients with acute pyelonephritis (APN) and of potential parenchymal scars found after a period of time from the acute event. DWI-MR (Diffusion-weighted magnetic resonance imaging) is a new and more sensitive method to detect inflammatory changes, including the kidney. The main aim of the study was detection of acute inflammatory changes in the renal parenchyma in children with acute pyelonephritis using the new DWI-MR method, determination of its sensitivity, and also comparison of the obtained results with the findings observed by static renal scintigraphy. Children over 3 years old have been chosen due to easier examination without need of general anesthesia or sedation. The second aim of the study was to determine whether this method can be applied in detection of chronic changes in the renal parenchyma after six months, determination of the extent and disposition of the chronic changes and their anatomical relation to acute lesions in comparison to DMSA-SRS.
