Rehabilitation after total endoprothesis of the right knee joint

Abstract:

Úvod: Název mé bakalářské práce je Rehabilitace po totální endoprotéze pravého kolenního kloubu. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky fyzioterapie u pacienta po totální endoprotéze kolenního kloubu. Metodika: Hlavní část práce je zaměřena na kazuistiku 73letého pacienta po totální endoprotéze pravého kolenního kloubu. Den před operací jsem provedla vstupní kineziologický rozbor (anamnéza, vyšetření aspekcí a palpací, antropometrie, goniometrie, vyšetření svalové síly dle Jandy, kloubní vůle, zkrácených struktur dle Jandy, pohybových stereotypů, chůze). Na základě vstupního kineziologického rozboru jsem vytvořila krátkodobý fyzioterapeutický plán. Fyzioterapeutickou intervenci jsem rozdělila do dvou částí. První část je zaměřena na rehabilitaci během hospitalizace od 26. 8. do 3. 9. 2020. Během hospitalizace provádím každý den kontrolní vyšetření se zaměřením na rozsah pohybu do flexe pravého kolenního kloubu, 5. a 8. den se zaměřením na otok pravé dolní končetiny a svalovou sílu pravého kolenního kloubu do flexe. Dále zde popisuji samotné terapie s pacientem v délce 45 minut. V druhé části se zabývám rehabilitací po ukončení hospitalizace od 16.9. do 29. 11. 2020. Rehabilitace probíhá formou návštěv u pacienta doma. Během 1., 2. a 3. návštěvy jsem se zaměřila na kontrolu rozsahu pohybu do flexe pravého kolenního kloubu, 1. a 2. návštěvu jsem dále kontrolovala otok pravé dolní končetiny. Dále jsou zde popsány samotné terapie s pacientem v délce 60 minut. Během terapií byly využity koncepty a metody jako senzomotorická stimulace, postizometrická relaxace, proprioceptivní neuromuskulární facilitace či Brügger koncept. 17. 1. 2021 jsem provedla výstupní kineziologický rozbor, na jehož základě jsem vytvořila dlouhodobý fyzioterapeutický plán. Výsledky: Následkem operace došlo ke korekci varózního postavení pravého kolenního kloubu. Během rehabilitace došlo k úpravě semiflekčního držení pravého kolenního kloubu z 10° na 2°, k redukci otoku celé dolní končetiny a k postupnému zvětšování hybnosti kolenního kloubu. Obnovil se správný stereotyp chůze bez jakékoliv bolesti. Závěry: Pacient byl díky rehabilitaci a aktivnímu přístupu k cvičení navrácen do běžného života.

Abstract:

Introduction: The title of my bachelor thesis is Rehabilitation after total endoprosthesis of the right knee joint. The objective: The aim of the bachelor thesis is to contribute to solving the problem of physiotherapy in the patientafter total knee arthroplasty. Methodology: The main part of the work is focused on the case report of a 73-year-old patient after total endoprosthesis of the right knee joint. The day before the operation, I performed an initial kinesiological analysis (anamnesis, examination of aspects and palpations, anthropometry, goniometry, examination of muscle strength according to Janda, joint will, shortened structures according to Janda, movement stereotypes, walking). Based on the initial kinesiological analysis, I created a short-term physiotherapy plan. I divided the physiotherapeutic intervention into two parts. The first part is focused on rehabilitation during hospitalization from August 26 to September 3 2020. During hospitalization, I perform daily check-ups focusing on the range of motion into the flexion of the right knee joint, days 5 and 8 with a focus on swelling of the lower right limbs and muscle strength of the right knee joint into flexion. I also describe the therapies themselves with a patient lasting 45 minutes. In the second part I deal with rehabilitation after hospitalization from September 16 to November 29 2020. Rehabilitation takes place in the form of visits to the patient at home. During the 1st, 2nd and 3rd visit I focused on checking the range of motion into the flexion of the right knee joint, 1st and 2nd visit I also checked the swelling of the right lower limb. Therapies of 60-minute patient therapy alone are also described. Concepts and methods such as sensorimotor stimulation, postisometric relaxation, proprioceptive neuromuscular facilitation and the Brügger concept were used during the therapies. On January 17 2021, I performed an output kinesiological analysis, on the basis of which I created a long-term physiotherapy plan. Results: As a result of the operation, the varus position of the right knee joint was corrected. During rehabilitation, the semiflective posture of the right knee joint was adjusted from 10 ° to 2 °, the swelling of the entire lower limb was reduced and the mobility of the knee joint was gradually increased. The correct stereotype of walking was restored without any pain. Conclusions: Thanks to rehabilitation and an active approach to exercise, the patient was returned to normal life.
