The influence of physiotherapy to foot load and back pain in pregnancy

Abstract:

Problematikou poklesu nožní klenby a změn postavení nohy v těhotenství, se již zabývalo množství studií. Tyto funkční poruchy mohou být zdrojem bolesti nebo se mohou generalizovat i do jiných oblastí PS. Výsledky některých studii prokazují přetrvávání změn v oblasti nohy i po porodu. Nedostatečně prozkoumanou oblastí je i přes vysokou prevalenci funkčních poruch u těhotných žen, jejich terapie. Cílem této diplomové práce je ověřit pozitivní vliv vybraných fyzioterapeutických postupů na eliminaci funkčních poruch. Zvolenou intervenci lze aplikovat již v průběhu těhotenství bez obavy poškození těhotné ženy nebo plodu. Výzkumu se zúčastnilo 12 zdravých těhotných žen, které byli randomizovaně rozděleny do výzkumného souboru a kontrolní skupiny. Výzkumný soubor tvořilo 6 probandek (průměrný věk 29,33 (?3,88) let, průměrná výška 161,7 (?9,56) cm, průměrná počáteční hodnota BMI 22,61 (?2,31)) a kontrolní skupinu tvořilo zbývajících 6 probandek (průměrný věk 29,83 (?3,19), průměrná výška 164,5 (?8,24) cm, průměrná počáteční hodnota BMI 23,66 (?4,86)). V každé skupině byl 50 % prvorodiček a 50 % vícerodiček. Pro získaní dat byl využit Dotazník McGillovy univerzity, Pregnancy mobility index, Foot Posture Index, Navicular Drop test. Výsledek: Nejvýznamnější změny se statisticky významným rozdílem (p?0,05) mezi vstupními a výstupními hodnotami byly zjištěny pouze u kontrolní skupiny těhotných žen. Z dotazníku bolesti se statisticky významně zvýšila hodnota VAS (p=0,04). Hodnota PMI z dotazníku interference bolesti s denními aktivitami se statisticky významně zvýšila (p=0,03), co poukazuje na sníženu kvalitu života. Změny v postavení nohy hodnocené FPI se statisticky významně zvýšili na PDK (p=0,04) i LDK (p=0,04). CSI hodnotící výšku nožní klenby, se statisticky významně zvýšil na PDK (p=0,03). U probandek výzkumného souboru nedošlo k statisticky významné změně v žádné hodnotě. Závěr: Intervence eliminuje vznik nebo rozsah funkčních poruch, ke kterým v těhotenství ženy inklinují - zvětšená pronace nohy, pokles nožní klenby a LBP. Z výsledků předpokládáme, že lze intervenci doporučit v ránci primární nebo sekundární prevence.

Abstract:

Several studies have already addressed the issue of foot arch decrease and changes in foot posture in pregnancy. These functional disorders may be a source of pain or may be generalized to other areas of PS. The results of some studies show persistence of changes in the foot postpartum. Despite the high prevalence of functional disorders of pregnant women, their therapy is explored insufficiently. The aim of this thesis is to verify the positive impact of selected physiotherapeutic procedures on the elimination of functional disorders. The intervention chosen can be applied during pregnancy without risk of damaging the pregnant woman or fetus. The research involved 12 healthy pregnant women who were randomized into a research group and a control group. The research group consisted of 6 probands (mean age 29.33 (? 3.88) years, mean height 161.7 (? 9.56) cm, mean BMI 22.61 (? 2.31)) and control group formed the remaining 6 probands (mean age 29.83 (? 3.19), mean height 164.5 (? 8.24) cm, mean BMI 23.66 (? 4.86)). In each group, 50 % were primiparous and 50 % were multiparous. To collect the data McGill University Questionnaire, Pregnancy Mobility Index, Foot Posture Index, Navicular Drop Test were used. Result: The most significant changes with a statistically significant difference (p?0.05) between entry and exit values were found only in the control group of pregnant women. From the pain questionnaire, VAS was significantly increased (p = 0.04). The PMI value from the questionnaire of interference pain with daily activities increased significantly (p = 0.03), therefore it indicates a decrease in quality of life. Changes in foot posture, evaluated by FPI, increased significantly on both extremities (p = 0.04). CSI evaluating the foot arch hight, increased significantly on right extremity (p = 0.03). There was no statistically significant change in any value for the probands of the research group. Conclusion: Intervention eliminates the emergence or extent of functional disorders that women tend to suffer from during pregnancy - increased pronation of the foot, decreased foot arch and LBP. The results suggest that the intervention can be recommended in early primary or secondary prevention.
