Quantitative methods of the detection of intrathecal IgM synthesis

Abstract:

Kvantitativní metody detekce intrathékální syntézy IgM. Lipowská Monika Lékařská fakulta Katedra biomedicínských oborů Syllabova 19 703 00 Ostrava 3 Tato bakalářská práce porovnává stanovení intrathékální syntézy IgM na analyzátoru BN ProSpec a na DSX analyzátoru. Principem měření na BN ProSpecu je nefelometrie a DSX analyzátor automatizuje testy ELISA. Teoretická část se zabývá likvorem, jeho vyšetřením od odběru až po stanovení důležitých parametrů. Další kapitola je věnována stanovení bílkovin, hlavně imunoglobulinu IgM. Nakonec je rozvedena intrathékální syntéza, její kvalitativní a kvantitativní stanovení a metody, kterými byla detekována. Praktická část popisuje vzorky, přístroje, kontrolní materiál a postupy. Na analýzu bylo použito 111 vzorků z rutinního provozu ÚKB FN Ostrava. Pro analýzu se používá likvor a sérum téhož pacienta, které je odebráno ve stejnou dobu jako likvor. Pro statistické zpracování byl použit Bland-Altmanův graf a Demingova regrese. U kontrolního materiálu byl použit T-test a F-test. Pomocí statistického zpracování výsledků bylo zjištěno, že ELISA technika vykazuje vyšší výsledky u stanovení IgM v séru, kdežto nefelometrie má vyšší výsledky u koncentrace IgM v likvoru a u kvocientu IgM.

Quantitative Methods of the Detection of Intrathecal IgM Synthesis. Lipowská Monika Fakulty of Medicine Department of Biomedical Sciences Syllabova 19 703 00 Ostrava 3 This bachelor´s thesis compares the determination of intrathecal IgM synthesis in BN ProSpec analyzer and DSX analyzer. The principle of measurement in the BN ProSpec analyzer is nephelometry and the DSX analyzer automates tests of ELISA. The theoretical part deals with cerebrospinal fluid, its collection and examination to determine important parameters. Another chapter is devoted to the determination of protein, mainly immunoglobulin M. Finally, the intrathecal synthesis is described, and its qualitative and quantitative determination of the methods, which has been detected. The practical part describes the samples, instruments, controls and procedures. The analysis was performed on the total 111 patients samples from routine operation at the Institute of Clinical Biochemistry of the University Hospital in Ostrava. Serum and cerebrospinal fluid is using from same patient for analysis, serum is removed at the same time as the cerebrospinal fluid. For statistical processing, Bland-Altman plot and Deming regression were used. T-test and F-test were used control material. With the statistical processing of the results, it was found, that the ELISA technique has higher scores for IgM in the serum, where as nephelometry results in a higher concentration of IgM in the cerebrospinal fluid and IgM quotient.
