Operation low positioned rectal carcinoma by the Rendes-vous technique, by the combination laparoscopic resection of the rektum and transanal endoscopic microsurgery using modified circular stapler.

Abstract:

Metoda operace nízce uloženého karcinomu rekta rendez - vous splňuje veškeré předpoklady onkologické radikality tím že umožňuje provedení resekce R0, tedy resekce s negativními resekčními plochami, umožňuje plnou přehlednou a bezpečnou totální mesorektální excizi, vytvořením mechanické koloanální anastomosy modifikovaným cirkulárním staplerem umožňuje odchod stolice per vias naturales při zachovalé kontinenci stolice. Mezi muži a ženami není po provedené operaci statistický rozdíl v manomentrické hodnotě musculus sfinkter ani externus, je zachována většinová kontinence plynů a plná kontinence stolice. Denní počet stolic je ustálen na hodnotě 3 – 4, rehabilitační program EMFB zlepšuje a urychluje návrat funkcí svěračového systému. Předoperačně provedená chemo-aktinoterapie statisticky neovlivní pooperační funkci svěračů. Není statisticky prokázán rozdíl v manometrických hodnotách.Mezi skupinou pacientů s tumorem lokalizovaným svým aborálním okrajem 1,5 – 2 cm od linea dentata a skupinou pacientů s tumorem s aborálním okrajem nad 2 cm nad linea dentata je rigorózně signifikantní statistický rozdíl v počtu aktivovaných uzlin mesorekta. Metoda rendez vous ve srovnání s literárními údaji splňuje podmínku totální radikální excize mezorekta a umístěním resekční linie v sulcus intersphinctericus ( v oblasti linea dentata ) rozšiřuje radikalitu totální mesorektáální excize. Potvrzuje též předpoklad, že tumory umístěné v etáži pod 2 cm aktivují v mesorektu více uzlin , než tumory nad 2 cm. Potvrzuje se tak dále předpoklad centrifugálního šíření tumorosního procesu v oblasti rekta. Mezi skupinou před provedením předoperační chemo-aktinoterapie není statisticky významného rozdílu v počtu aktivovaných uzlin mesorekta. Mezi stádiem T2 a T3 tumoru rekta není statisticky významného rozdílu v počtu aktivovaných uzlin mesorekta. Metoda Rendez – vous přináší inovaci v technice T.E.M. Tato inovace spočívá v zavedení krátkého tubusu bez zešikmení pracovního konce, který umožňuje provedení cirkulárního protětí rekta a bezpečnou a přehlednou preparaci perirektální.Metoda Rendez – vous přináší též nové technické řešení provedení mechanické kolo - anální anastomózy. Zavádí modifikovaný cirkulární stapler, jehož pomocí lze bezpečně provést cirkulární kolo – anální anastomózu v oblasti linea dentata a kdekoliv pod její úrovní.

Abstract:

The operational method by the Rendez-vous technique at low positioned rectal carcinoma fulfills all conditions of oncological radicality by allowing the performance of R0 resection, that is the resection with negative resectional planes; it allows full panoramic and safe total mesorectal excision; by creating mechanical coloanal anastomosis using modified circular stapler it allows defecation per vias naturales preserving dejection continence. There is no statistical difference between the manometric values of musculus sphincter ani externus at male and female patients after the surgery, major continence of gasses and full continence of feces is preserved. The daily number of defecations is fixed at 3 - 4, the EMFB rehabilitation programme improves and accelerates the recovery of the sphincter system functions. Preoperationally performed chemoactinotherapy will not statistically influence postoperational sphincter function. The difference between manometric values was not proven statistically. There is rigorously significant statistical difference between the number of activated mesorectal nodes in the group of patients with the tumor with aboral margin localized 1,5 - 2 cm from the linea dentata and those with the tumor´s aboral margin farther than 2 cm from the linea dentata. The Rendez-vous method compared with the data from the literature fulfills the condition of total radical excision of mesorectum and by placing the resection line in the sulcus intersphinctericus (at the linea dentata region) extends the radicality of total mesorectal excision. It also confirms a presumption that tumors localized at the area under 2 cm activate more nodes in the mesorectum than those over 2 cm. It further confirms the presumption about centrifugal diffusion of tumorous process in the rectal area. There is no statistically significant difference between the number of activated mesorectal nodes in the group with preoperational chemoactinotherapy. There is no statistically significant difference between the number of activated mesorectal nodes in the T 2 and the T 3 rectal tumor stage. The Rendez-vous method brings inovation to the T. E. M. technique. This inovation consists in the introduction of a short tubus without the inclination of the working end, that allows to perform circular sphincterectomy and safe perirectal preparation with a clear overview of the operational field. The Rendez-vous method also brings a new technical solution of performing mechanical coloanal anastomosis. It introduces modified circular stapler and with its help it is possible to safely perform circular coloanal anastomosis at the area of the linea dentata and anywhere under it.
