Distal crural fractures - possibilities of surgical treatment

Abstract:

Cíl: Zhodnocení souboru pacientů léčených pro zlomeninu distálního bérce, optimalizovat léčebnou strategii a taktiku ošetřování těchto pacientů formou prospektivní studie. Materiál a metodika: Soubor operovaných pacientů s intraartikulární zlomeninou distální tibie, dle klasifikace AO B a C typ zlomenin. V tomto souboru bylo 50 mužů a 26 žen. Výsledky souboru byly hodnoceny podle hojení měkkých tkání, rtg podle podle Burwella, spokojenost pacienta a klinický výsledek podle skórovacího systému AOFAS. Z celkového počtu bylo 37 pacientů akutně stabilizováno zevním fixatérem s pozdější konverzí na vnitřní osteosyntézu. Nejčastějším způsobem definitivního ošetření zlomenin distální tibie byla otevřená repozice a vnitřní fixace (ORIF) - 69 zlomenin. Ke stabilizaci byly využívány úhlově stabilní implantáty, anatomicky tvarované dlahy na distální tibii, mediální nebo anterolaterální dlaha, u 7 pacientů byla provedena miniosteosyntéza. Výsledky: V rámci klinického či funkčního hodnocení dosaženého léčebného efektu dle skórovacího systému AOFAS bylo 14 výborných výsledků, 26 dobrých, 24 uspokojivých a 12 špatných výsledků. Porovnáním 2 skupin pacientů bylo zjištěno, že použití zevní fixace při akutní stabilizaci zlomeniny a následná konverze na vnitřní fixaci statisticky významně snižuje riziko vzniku pooperačních ranných komplikací u C typu zlomenin. Závěr: Otevřená repozice fragmentů a následná vnitřní fixace pomocí úhlově stabilních dlahových implantátů je plně indikovaná metoda operační léčby nitrokloubních zlomenin distální tibie. Komplikované vysokenergetické zlomeniny C typu je lépe akutně stabilizovat zevním fixatérem a konverzi na vnitřní fixaci provést s odstupem, po zlepšení stavu měkkých tkání. Abstract:

Objective: Evaluation of patients treated for a fracture of the distal tibia. The aim was to optimize treatment strategy and tactics of treatment of these patients using a prospective study. Material and methods: Compilation of surgically treated patients with intra-articular fractures of the distal tibia, according to AO classification type B and C fractures. There were 50 men and 26 women in this group. The results were evaluated according to the healing of a soft tissue, according to Burwell X-ray and as well to patient satisfaction. Clinical outcome was classified according to AOFAS scoring system. Overall 37 patients were acutely stabilized using external fixator with later conversion to internal osteosynthesis. The most common way of definitive treatment of fractures of the distal tibia was the open reduction and internal fixation (ORIF) - 69 fractures. For the stabilization, there were used the angular stable implants, anatomically shaped distal tibia plates, medial or anterolateral plates. 7 patients were treated with miniosteosynthesis. Results: The clinical and functional assessment of therapeutic effect according to AOFAS scoring system was excellent in 14 cases, good in 26, satisfactory in 24 and unsatisfactory in 12 cases. Comparing the two groups of patients, it was found that the use of external fixation in acute fracture and subsequent conversion to internal fixation significantly reduces the risk of early postoperative complications in type C fractures. Conclusion: Open reduction of fragments and subsequent internal fixation with angular stable splints is fully indicated method of operative treatment of intraarticular fractures of the distal tibia. Complicated high energy type C fractures are more acutely stabilized by external fixator and conversion to internal fixation performed after improvement of the soft tissues.
