Detection and biopsy of the sentinel lymph node in malignant tumours

Abstract:

Sentinelová uzlina je první uzlinou, která drénuje lymfatický tok z primárního nádoru. Je-li tato uzlina postižena, lymfatické metastazování proběhlo. Není-li však zasažena, je vysoce pravděpodobné, že tumor dosud lymfogenně nemetastazoval. Hlavním cílem detekce a biopsie sentinelové uzliny je stanovení přesného stádia onemocnění identifikací metastatického postižení uzlin a vyhnutí se tak negativnímu dopadu lymfadenektomie na kvalitu života pacienta. K detekci sentinelové uzliny se v současné době používají tři základní techniky - předoperační lymfoscintigrafie, peroperační použití modré barvy a gamasondy. Předmětem bakalářské práce bylo vzájemné porovnání detekční úspěšnosti sentinelové uzliny mezi předoperační lymfoscintigrafií na Klinice nukleární medicíny a peroperačně navigovanou chirurgií s použitím patentní modři na jiných spolupracujících pracovištích.

Abstract:

Sentinel lymph node is the first node draining the lymphatic flow from primary tumour. If this node is affected, there were lymphatic metastases. If, however, it is not affected, it is most likely that the tumour has not create lymphatic metastases yet. The main objective of detection and biopsy of sentinel lymph node is determination of exact stage of disease by identification of nodes with lymphatic metastases, thus avoiding the negative impact of lymphadenectomy on the patient?s quality of life. Three basic techniques are currently used to detect the sentinel lymph node ? pre-operation lymphoscintigraphy, per-operation use of blue dye, and gamma-probes. The subject of the bachelor thesis is mutual comparison of detection success rate of the sentinel node using the pre-operation lymphoscintigraphy at the Clinic of Nuclear Medicine and using the per-operation navigated surgery with patent blue at other co-operating centres.
