Healthcare-associated infections caused by Clostridioides difficile

Abstract:

Tato bakalářská práce se zabývá problematikou infekcí spojených se zdravotní péčí způsobených Clostridioides difficile. Clostridioides difficile je gram pozitivní anaerobní bakterie, která má schopnost vytvářet velice odolné spory a způsobuje infekce gastrointestinálního traktu. První příznaky se nejčastěji projevují ve spojitosti s širokospektrálními antibiotiky. Mezi typické symptomy infekce způsobené Clostridioides difficile patří vodnaté průjmy s hnilobným zápachem a křeče v břiše. Mezi faktory, které zvyšují riziko vzniku infekce patří antimikrobiální léčba, která narušuje střevní mikroflóru, s tím spojená dlouhodobá hospitalizace a věk pacientů. Mezi nejvíce rizikovou věkovou skupinu patří pacienti od 61 do 80 let. Důležitou součástí této problematiky jsou preventivní procesy, které slouží k zabránění šíření infekcí ve zdravotnickém zařízení. Měla by být dodržována správná hygiena rukou se správně zvoleným dezinfekčním prostředkem, z důvodu odolnosti spor vůči běžným alkoholovým dezinfekcím. Dalším preventivním opatřením je izolace infekčního pacienta s povinností ošetřujícího personálu používat osobní ochranné pracovní pomůcky. Léčba infekcí způsobených Clostridioides difficile se léčí vhodně zvolenou antimikrobiální léčbou, v případě neúčinnosti léčby z důvodu antimikrobiální rezistence nastává léčba pomoci transplantace stolice. Provedeného výzkumu se zúčastnilo 343 respondentů, kteří byli v letech 2020-2021 hospitalizováni ve Fakultní nemocnici Ostrava a byla u nich prokázána pozitivita na bakterii Clostridioides difficile. Bylo zjištěno, že infekce spojené se zdravotní péčí se vyskytují častěji u mužů než u žen. Následně bylo potvrzeno, že vyšší věk pacientů je velice rizikový pro vznik infekcí a s tím i souvisí nejčastější záchyt infekcí spojených se zdravotní péčí způsobených Clostridioides difficile, po přepočtu na počet hospitalizovaných pacientů, který byl zaznamenán v Léčebně dlouhodobě nemocných v Klokočově.

Abstract:

This Bachelor thesis deals with the issue of healthcare associated infections caused by Clostridioides difficile. Clostridioides difficile is a gram-positive anaerobic bacterium that has the ability to produce highly resistant spores and causes gastrointestinal infections. The first symptoms are most commonly seen in association with broad spectrum antibiotics. Typical symptoms of infection caused by Clostridioides difficile include watery diarrhea with a rotting odour and abdominal cramps. Factors that increase the risk of getting an infection include antimicrobial therapy that disrupts the gut microflora, associated long-term hospitalisation and patient age. Patients aged between 61 and 80 are among the most at-risk age groups. Preventative processes to prevent the spread of infection in health facilities are an important part of this issue. Good hand hygiene with the right choice of disinfectant should be followed, for reasons of resistance to common alcohol disinfectants. Another preventive measure is the isolation of an infectious patient with the duty of nursing staff to use personal protective work equipment. Treatment of infections caused by Clostridioides difficile is treated with appropriately chosen antimicrobial therapy, in case of ineffectiveness of treatment due to antimicrobial resistance, treatment occurs with the help of faecal transplantation. The research was carried out by 343 respondents who were hospitalised at the University Hospital Of Ostrava in 2020-2021 and were shown to be positive for the bacterium Clostridioides difficile. Healthcare-associated infections were found to occur more frequently in men than women. Subsequently, it was confirmed that the higher age of patients is very risky for developing the infection and this is linked to the most frequent interception of healthcare associated infections caused by Clostridioides difficile, after conversion to the number of hospitalized patients seen in the Klokočov Long-Term Hospital.
