Laboratory Monitoring of Treatment Using Extended Half-Life Coagulation Factor Concentrates in Patients with Haemophilia

Abstract:

Hemofilie A a B jsou dědičné krvácivé choroby způsobené deficitem koagulačního faktoru VIII, resp. IX. Standardní léčba spočívá v substituci chybějícího koagulačního faktoru v podobě koncentrátů. Během posledních let byly do praxe zavedeny nové koncentráty koagulačních faktorů, jejichž molekula byla upravena tak, aby se prodloužila jejich clearance. Nespornou výhodou těchto preparátů s prodlouženým biologickým poločasem (EHL) je menší zátěž pacientů spočívající v méně častých intravenózních aplikacích, avšak modifikace molekul FVIII/FIX vedly k obtížím při laboratorním monitorování. V současné době existují dvě komerčně dostupné metody pro měření aktivity FVIII/FIX – jednofázová koagulační metoda na bázi APTT a dvoufázová metoda s chromogenními substráty. Výsledky několika studií prokazují problémy s měřením správných hladin FVIII/FIX u EHL preparátů pomocí jednofázové koagulační metody, které souvisejí s APTT reagencií, respektive typem použitého aktivátoru. Cílem bakalářské práce je porovnat výsledky aktivity FVIII/FIX získané jednofázovou koagulační metodou a dvoufázovou metodou s chromogenními substráty u pacientů léčených EHL koncentráty

Abstract:

Haemophilia A and B are inherited bleeding disorders caused by deficiency of coagulation factor VIII and factor IX, respectively. The standard treatment is to replace missing clotting factor in the form of concentrates. In recent years, new clotting factor concentrates have been introduced into practice, the molecule of which has been modified to extend their clearance. An indisputable advantage of these extended half-life (EHL) factor concentrates is the lower burden on patients due to less frequent intravenous applications. Modifications of FVIII/FIX molecules have led to difficulties in laboratory monitoring. There are currently two commercially available methods for measuring FVIII/FIX activity – the one-stage APTT-based clotting assay and the two-stage chromogenic substrate assay. The results of several studies show problems with measuring the correct levels of FVIII/FIX in EHL concentrates using one-stage clotting assay, which are related to the APTT reagent and the type of activator used. The aim of the bachelor thesis is to compare the results of FVIII/FIX activity obtained by the one-stage clotting assay and two-stage method with chromogenic substrates in patients treated with EHL concentrates.
