Neuropsychological profile of cognitive functions in mental disorders ( comparison between schizoaffective and bipolar affective disorder)

Abstract:

Cílem této práce je zmapovat a porovnat profily kognitivních funkcí u pacientek se schizoafektivní (n=19) a bipolární afektivní (n=19) poruchou a subtypů obou diagnóz. Zařazeno je také srovnání s kontrolní skupinou odpovídající věkem, pohlavím a vzděláním výzkumnému vzorku. Kognitivní funkce byly zmapovány pomocí Paměťového testu učení, Testu cesty (část A a B), Wechslerova subtestu Opakování čísel, Testem verbální fluence, Testem kognitivního odhadu, Bentonovým vizuálním retenčním testem a Stroopovým testem. Rozdíly mezi skupinami jsou určeny statistickou analýzou pomocí Mann-Whitneyho U testu. Z výsledků vyplývá, že bipolární afektivní a schizoafektivní pacientky vykazují deficity v doménách pozornosti, paměti, exekutivních funkcí a psychomotorického tempa v porovnání s kontrolní skupinou i s normami populace. Nebyly zjištěny žádné statisticky významné rozdíly mezi bipolární a schizoafektivní skupinou pacientek. U jednotlivých subtypů byl nalezen statisticky významný rozdíl (p<0,5) pouze ve výsledku Testu verbální fluence, kde lepších výsledků dosahoval depresivní subtyp schizoafektivní poruchy. Z výsledků srovnávacích analýz vyplývá, že profil kognitivních funkcí není vhodným kritériem pro rozlišení schizoafektivní a bipolární afektivní poruchy.

Abstract:

The aim of this thesis is to map and compare cognitive function profiles in patients with schizoaffective (n = 19) and bipolar affective (n = 19) and subtypes of both diagnoses. Also included is a comparison with the control group corresponding to the age, sex, and education of the research sample. Cognitive functions were mapped using the Auditory Verbal Learning Test, Trail Making Test (Parts A and B), Wechsler Digit Span, Verbal Fluency Test, Cognitive Estimation Test, Benton Visual Retention Test and finally Stroop Test. Differences between groups were determined by statistical analysis using the Mann-Whitney U test. The results show that bipolar affective and schizoaffective patients show deficits in the attention, memory, executive, and psychomotoric pace compared to both the control group and population standards. No statistically significant differences were identified between the bipolar and schizoaffective groups of patients. For each subtype, a statistically significant difference (p<0.5) was found only in the result of the Verbal Fluence Test, where the depressive subtype of schizoaffective disorder was superior. The results of the comparative analysis show that the cognitive profile is not a suitable criterion for distinguishing schizoaffective and bipolar affective disorder.
