Abstract:

Candida albicans je nejčastějším původcem povrchových i invazivních kandidóz a je to významný nozokomiální patogen. V posledních letech však nabývají na zvýšeném klinickém významu i tzv. druhy non-albicans. Mezi nimi také druh sdílející s C. albicans mnoho fenotypických charakteristik, a to C. dubliniensis. Předmětem mnoha prací jsou možné způsoby odlišení C. dubliniensis od C. albicans a usnadnění identifikace této kvasinky v klinických laboratořích. V této práci jsme se zabývali identifikací C. dubliniensis izolovaných z běžného klinického materiálu se zaměřením především na stolici. Kromě toho jsme posuzovali spolehlivost tří různých fenotypických identifikačních metod, jejichž výsledky byly srovnávány s výsledky získanými z latexové aglutinace (Bichro-Dubli Fumouze) a byly ověřeny pomocí PCR. Nejvyšší stupeň spolehlivosti prokázalo použití minimálně dvou fenotypických metod, při použití všech tří metod byly výsledky až na několik falešně pozitivních téměř identické s latexovou aglutinací. Je tedy doporučeno brát fenotypické testy pouze jako orientační a identifikaci potvrdit nejlépe pomocí molekulárně biologických metod. Byl prokázán výskyt C. dubliniensis v běžném klinickém materiálu s incidencí 2,8 %. Kromě ústní dutiny a dýchacích cest se podařilo izolovat tuto kvasinku také ze stolice, což je zajímavé, protože mnoho autorů se tímto klinickým materiálem v souvislosti s uvedeným druhem doposud nezabývalo.

Abstract:

Candida albicans is the most frequent isolated species of genus Candida and the main etiological agens of superficial and invasive candididasis. It´s also an important nosocomial pathogen. Recently other species than C. albicans have increased their incidence and clinical importance as well. One of them is C. dubliniensis sharing many phenotypical settings with C. albicans. As a result many studies are trying to find out the way of distinguishing between these two species and the way of identifying it in common clinical laboratories. The aim of this study was isolation and identification of this yeast from different kinds of clinical specimen and comparation the reliability of three phenotypical methods used for its identification. These findings were compared with latex agglutination and were verified by polymerase chain reaction. Usage of at least two of the phenotypic methods was found reliable, usage of three methods was, apart from a couple of falsely positive ones, comparable to latex agglutination. Therefore it is recommended to confirm these tests by molecular biological mehods. We proved the incidence of C. dubliniensis in clinical specimen with frequency of 2,8 %. Besides oropharynx and airways the main source of this yeast was a stool. It´s an interesting finding, especially due to the fact that only small number of publications was focused on this kind of clinical specimen.
