Quality of life of elderly with osteoarthrosis of the hip joint

Abstract:

Cíl: Cílem diplomové práce bylo zjistit kvalitu života seniorů s osteoartrózou kyčelního kloubu. Zjistit, zda existuje rozdíl v subjektivním vnímání kvality života s osteoartrózou kyčelního kloubu mezi seniory dle věku, mezi muži a ženami, v souvislosti, zda jsou v partnerském vztahu, či nejsou. A také, zda existuje souvislost mezi subjektivně vnímanou mírou bolesti, mírou ztuhlosti a vnímání obtížích při běžných denních aktivitách a kvalitou života. Metodika: Do výzkumného souboru bylo zařazeno 111 seniorů starších 65 let hospitalizovaných na ortopedických odděleních s osteoartrózou kyčelního kloubu I. až III. stupně s výsledky Montrealského kognitivního testu minimálně 26 bodů. Ze 120 rozdaných dotazníků 5 respondentů nevyplnilo dotazníky v celém rozsahu a 4 respondenti měli výsledky Montrealského kognitivního testu méně než 26 bodů. Pro získání potřebných údajů byl zvolen standardizovaný dotazník SF-36 (Short Form36) a dotazník Western Ontario & McMaster Universities Osteoarthritis Index (WOMAC Index). Výsledky: Z výzkumu vyplynulo, že horší kvalitu života ve všech oblastech mají starší senioři ve věku 71-80 let oproti seniorům ve věku 65-70 let. Nepatrně menší subjektivní kvalitu života vnímají muži, zatímco ženy vnímají své zdraví lépe. Největší změny v subjektivním vnímání kvality života hodnotili svobodní respondenti. Primární srovnání dotazníků SF-36 a WOMAC neukázalo souvislost mezi subjektivně vnímavou mírou bolesti a kvalitou života, avšak otázka z dotazníku SF-36, která se zaměřovala na bolest prokázala, že jsou subjektivně vnímaná míra bolesti a kvalita života na sobě silně závislé, přičemž s rostoucí jednou hodnotou roste i druhá. V souvislosti mezi subjektivně vnímanou mírou ztuhlosti, obtížích při běžných denních aktivitách a kvalitou života nebyla v tomto výzkumu shledána významná souvislost Závěr: Na základě zjištěných skutečností vyplývá, že osteoartróza kyčelního kloubu ovlivňuje kvalitu života seniorů, proto je důležité, aby se u těchto pacientů pravidelně monitorovala kvalita života a aby byl kladen důraz na individuální přistup a subjektivní vnímání daného seniora.

Abstract:

Objective: The aim of the thesis was to assess the quality of life of seniors with osteoarthritis of the hip joint. Additionally, the objective was to determine whether there are differences in the subjective perception of quality of life among seniors with osteoarthritis of the hip joint based on factors such as age, gender, relationship status, and whether they experience pain, stiffness, or difficulties in normal daily activities. Methodology: A total of 111 seniors aged over 65, hospitalized in orthopedic departments with osteoarthritis of the hip joint (grades I to III), and scoring at least 26 points on the Montreal Cognitive Test were included in the research group. Out of 120 distributed questionnaires, 5 were incomplete, and 4 respondents scored below 26 points on the Montreal Cognitive Test. The standardized SF-36 (Short Form-36) questionnaire was selected to collect the necessary data, along with the Western Ontario & McMaster Universities Osteoarthritis Index (WOMAC Index) questionnaire. Results: The research showed that older seniors, aged 71-80, have a worse quality of life in all areas compared to seniors aged 65-70. Men tended to perceive a slightly lower subjective quality of life compared to women. Unmarried respondents rated their subjective quality of life the highest. While a primary comparison of the SF-36 and WOMAC questionnaires did not reveal associations between the subjectively perceived level of pain and quality of life, a specific question from the SF-36 questionnaire focusing on pain indicated a strong interdependence between the subjectively perceived level of pain and quality of life, where an increase in one value also increases the other. Furthermore, the research also demonstrated that there is no relationship between the subjectively perceived degree of stiffness, difficulties in normal daily activities, and quality of life. Conclusion: Based on the findings, it is evident that osteoarthritis of the hip joint significantly impacts the quality of life of seniors. Therefore, it is crucial to regularly monitor the quality of life of these patients and prioritize individualized approaches that consider their subjective perceptions.
