Validation of the nursing diagnosis anxiety of women with malignant breast disease

Abstract:

Cílem mé bakalářské práce je validizace ošetřovatelské diagnózy úzkost u žen s maligním onemocněním prsu (stav po operaci) pomocí Ferhlingova modelu - Na pacienta zaměřen model klinické diagnostické validity. Mým záměrem je zjistit míru úzkosti u těchto žen a poté určit pomocí váženého skóre hlavní, vedlejší a podřadné charakteristiky. Ke splnění tohoto cíle jsem si stanovila následující úkoly práce. Vyhledat skupinu žen po operaci prsu, které mají v ošetřovatelské dokumentaci stanovenou ošetřovatelskou diagnózu úzkost. Rozdat těmto ženám Zungův sebeposuzovací test k potvrzení správně stanovené diagnózy úzkost. Validizace diagnózy úzkost pomocí Ferhlingova modelu - Na pacienta zaměřen model klinické diagnostické validity a posouzení jednotlivých charakteristik. Vyřadit z výzkumu ženy, u kterých se ošetřovatelská diagnóza úzkost nepotvrdila. Nástrojem ke zjištění informací o zkoumané problematice byl použit anonymní dotazník (viz. Příloha A). Dotazník byl rozdán pacientkám po operaci prsu, u kterých byla v ošetřovatelské dokumentaci stanovena ošetřovatelská diagnóza úzkost. Dotazník byl použit na chirurgickém oddělení v Chomutově a Ústí nad Labem. Dotazníků bylo rozdáno celkem 150, z toho se jich vrátilo 52. Ne každá žena se chtěla na studii podílet v tak těžké situaci. Pomocí Zungovo sebeposuzovacího testu k potvrzení správně stanovené diagnózy úzkost jsem vyřadila 5 dotazníků. Poté jsem pomocí Ferhlingova modelu - Na pacienta zaměřen model klinické diagnostické validity určila hlavní, vedlejší a podřadné charakteristiky. Mezi hlavní charakteristiky patří strach z nespecifikovaných důsledků, pocity neadekvátnosti, stres, snížená schopnost se učit, úzkostlivost, snížená schopnost řešit problémy, přehnané zaujetí něčím, strach, obavy kvůli změně životních událostí, snížená produktivita, potíže s koncentrací a neklid. Mezi vedlejší charakteristiky patří dalších 28 charakteristik. Z výsledků vyplívá, že ze zvolených 44 charakteristik je jich 12 považováno za hlavní charakteristiky a 28 za vedlejší charakteristiky.

Abstract:

The object of my bachelor thesis is a validation of the nursing diagnoses anxiety of women with malignant breast cancer (after surgery). I am using Ferhling model - Patient - focused CDV model. My intention is to determine the degree of anxiety of these women and determine using the weighted of primary, secondary and inferior characteristics. I have identified the following tasks. A group of women after breast surgery and referenced nursing diagnosis anxiety in nursing documentation. The Zung test was distributed to these women to confirm the correct diagnosis anxiety. Validation of the diagnosis of anxiety using Ferhling model - Patient - focused CDV model and assessment of individual characteristics. And to exclude women from the research where anxiety was not confirmed. To get information an anonymous questionnaire was used. The questionnaire was distributed to patients after mastectomy, in which the nursing documentation had anxiety in the nursing diagnosis. The questionnaire was used in the surgical ward in Chomutov and Ústí nad Labem. 150 questionnaires were distributed and 52 were returned. Not every woman wanted to participate in the study during the difficult situation. The Zung self-evaluation test is used to confirm the correct diagnosis of anxiety. I excluded 5 questionnaires. Then I'm using Ferhling model - Patient - focused CDV model identified the main, secondary and inferior characteristics. The main characteristics include unspecified fear of consequences, feelings of inadequacy, stress, reduced ability to learn, anxiety, reduced ability to solve problems, excessive preoccupation with something, fear, worry due to the change of life events, reduced productivity, trouble with concentration and restlessness. The secondary characteristics include 28 other characteristics. The results imply that out of the 44 selected characteristics 12 are considered to be main charakter and 28 to be secondary.
