Transfer musculus flexor hallucis longus u rozsáhlých ruptur Achillovy šlachy

Musculus flexor hallucis longus transfer in treatment of neglected Achilles tendon rupture

Abstract:

Ošetřili jsme 20 pacientů s poraněním Achillovy šlachy 2 incizní operační technikou podle Wapnera – transferem šlachy flexor hallucis longus. Modifikovali jsme způsob ukotvení šlachy do patní kosti. Pacienty jsme sledovaly 1 rok od operace. AOFAS Hindfoot score vzrostlo z původní předoperační průměrné hodnoty 27 k výsledné průměrné hodnotě 93. S celkovým výsledkem operace bylo spokojeno 15 pacientů. Modifikovanou Wapnerovu techniku transferu šlachy flexor hallucis longus lze považovat za vhodnou alternativu pro řešení rozsáhlých ruptur Achilovy šlachy. V průběhu několika operací došlo opakovaně k diskrepanci mezi lokálním nálezem a běžně popisovanou anatomickou strukturou v oblasti křížení šlach flexor hallucis longus a flexor digitorum longus. Zkoumali jsme teda tuto oblast na kadaverech a hledali jsme orientační bod na noze, vzhledem k existujícím anatomickým strukturám, vhodný pro miniinvazivní preparaci Henryho uzlu. Ve střední části nohy existuje propojení šlach musculus flexor hallucis longus a musculus flexor digitorum longus ve formě spojky od šlachy flexor hallucis longus k distálnímu větvení šlachy flexor digitorum longus pro druhý nebo i třetí prst. Nejedná se o pouhé křížení těchto šlach. Při preparaci „Henryho uzlu“ se můžeme spolehnout, že v 95 % případů se nachází přibližně v polovině vzdálenosti mezi linií I. metatarsophalangeálního kloubu a hrbolem patní kosti.

Abstract:

We treated 20 patients with tear of the Achilles tendon by modified two incisions surgical procedure according to Wapner – flexor hallucis longus tendon transfer. We changed the method of the fixation transferred tendon to the calcaneus. Follow up 6 months. The AOFAS Hindfoot score outcomes grown from average 27 to 93 in 1 year after the surgery. Subjectively, 15 patients were satisfied with final effect of the surgery. Modified Wapner technique of flexor hallucis longus tendon transfer is sufficient alternative for treatment of neglected Achilles tendon rupture. During the surgeries we noticed a discrepancy between local structure and the standardly described anatomical relationship of flexor hallucis longus and flexor digitorum longus tendons. We studied this structure on cadaver legs and tried to find a landmark on the foot due to the existing anatomical structures for minimally invasive preparation of the Henry`s knot. There was always a tendinous slip branching from the flexor hallucis longus tendon distal to the flexor digitorum longus tendon for the second toe or for the second and the third toe in the middle part of the foot. So, this is not only crossing of tendons. Henry`s knot is located approximately halfway between the line I. metatarsophalangeal joint and the tuber calcanei in 95% of cases.
