Neurodegenerative brain diseases: Neural correlates of clinical symptoms and therapeutic intervention

Abstract:

Parkinsonova choroba (PD) je neurodegeneratívne ochorenie, ktoré postihuje milióny ľudí na celom svete a je spojené s rôznymi motorickými a nemotorickými príznakmi. Doposiaľ neexistuje liečebný postup, ktorým by bolo možné vyliečiť alebo natrvalo zastaviť progresiu tohto ochorenia. Zamerali sme sa na štrukturálne aj funkčné MRI zmeny u subtypov PD, so špeciálnym zameraním na identifikáciu neurálnych korelátov symptómov, ktoré sú obtiažne liečiteľné majú veľký dopad na kvalitu života, najmä na kognitívnu poruchu a hypokinetickú dysartriu. Cieľom tohto výskumu bolo identifikovať špecifické štrukturálne a funkčné zmeny u PD pacientov spojené s určitými kognitívnymi subtypmi. Ďalším cieľom bolo ohodnotiť neurálne koreláty hypokinetickej dysartrie (HD), častého symptómu PD, a preskúmať efekty repetitívnej transkraniálnej magnetickej stimulácie (rTMS) na HD. Najprv sme skúmali štruktúru mozgu a funkčnú konektivitu u PD pacientov s využitím rôznych metód. Pre skúmanie atrofie šedej hmoty (GM) súvisiacej s rôznymi kognitívnymi subtypmi PD sme použili „voxel-based“ morfometriu, „source-based“ morfometriu a analýzu hrúbky kortexu. Pri hodnotení zmien vo funkčnej konektivite veľkých mozgových sietí sme využili „seed-based“ korelačnú analýzu, analýzu „partial least squares“ a analýzu sietí pomocou grafovej teórie v prospektívnej longitudinálnej štúdii PD pacientov a vekovo viazaných zdravých kontrol (HC). V druhej časti našej práce sme skúmali neurálne koreláty kontroly reči pomocou hodnotenia objemov šedej hmoty v oblastiach, ktoré súvisia s rečou, a funkčnej konektivity relevantných štruktúr počas čítacej úlohy v MR skeneri. Navyše sme cielili na identifikáciu optimálnych kortikálnych oblastí pre rTMS terapiu pre zlepšenie problémov motorickej reči u PD pacientov. Na dosiahnutie tohto cieľa sme skúmali krátkodobé efekty repetitívnej transkraniálnej magnetickej stimulácie (rTMS) aplikovanej nad ľavú orofaciálnu primárnu motorickú oblasť, pravý gyrus temporalis superior posterior (STG) a nad vertex (kontrolná oblasť) s využitím vysokofrekvenčnej a nízkofrekvenčnej rTMS. U štúdie s kognitívnymi subtypmi PD sme zistili, že PD s miernou kognitívnou poruchou (PD-MCI) mali oproti PD s normálnou kogníciou (PD-NC) väčšiu atrofiu šedej hmoty v orbitofrontálnych oblastiach („voxel-based“ morfometria), ktorá súvisela so z-skórami pamäťových funkcií, a v ľavom lobulus parietalis superior (hrúbka kortexu); rozšírenejšia atrofia v limbických a fronto-parieto-okcipitálnych neokortikálnych oblastiach (všetky metódy) odlíšila túto skupinu od HC. Iba analýza hrúbky kortexu odhalila drobnú atrofiu v prednom cingulu, precuneu a v temporálnom neokortexe u PD-NC v porovnaní s HC, ale tieto zmeny nekorelovali so žiadnymi kognitívnymi skórami. V štúdii funkčnej konektivity, medzisieťová konektivita frontoparietálnej kontrolnej siete a iných kľudových sietí bola znížená u PD-NC, a analýza v rámci celého mozgu odhalila zvýšenú charakteristickú dĺžku cesty a zníženú priemernú silu uzla, klastrovací koeficient a globálnu účinnosť u PD-NC v porovnaní s HC. Hodnoty všetkých parametrov u PD-MCI sa nachádzali medzi hodnotami u HC a PD-NC. Po jednom roku sa medzisieťová konektivita ďalej zvýšila u zmiešanej skupiny PD, v skupine HC sme nepozorovali žiadne zmeny. V štúdii o motorickej reči PD pacienti v porovnaní s HC prejavili zníženie hlasitosti reči v čase, ktoré súviselo so zvýšením BOLD signálu v pravostranných oblastiach dorzálnej jazykovej dráhy/ artikulačnej siete. Spomedzi týchto oblastí v porovnaní s HC bola u PD zvýšená aktivácia predného cingula. kou iregularitou v PD skupine. V exploratívnej štúdii rTMS, 1 Hz stimulácia spôsobila signifikantný nárast relatívnej štandardnej odchýlky druhého formantu, t.j. akustický parameter, ktorý popisuje kvalitu artikulácie. Efekty boli väčšie ako pri stimulácii kontrolnej oblasti a boli sprevádzané zvýšenou kľudovou funkčnou konektivitou medzi stimulovanou oblasťou a pravým gyrus parahippocampalis. V štúdii s kognitívnymi subtypmi PD boli pre identifikáciu neurálnych korelátov PD-MCI v porovnaní s PD-NC a HC senzitívnejšie „voxel-based“ morfometria a analýza hrúbky kortexu ako „source-based“ morfometria. Hlavné rozdiely boli pozorované vo frontálnych limbických a horných parietálnych oblastich, ktoré sú postihnuté pri pamäťových deficitoch a deficitoch v pozornosti/exekutívnych funkciách, ktoré sú charakteristické pre PD-MCI. Iba analýza hrúbky kortexu odhalila jemné rozdiely medzi HC a PD-NC, a teda by mohla potenciálne slúžiť ako skorý marker PD. Štúdia funkčnej konektivity odhalila, že v porovnaní s HC už PD-NC pacienti vykazujú globálne menej efektívny transport informácií a zníženú medzisieťovú konektivitu, hlavne u vizuálnej a frontoparietálnej kontrolnej siete. Medzisieťová konektivita sa zvyšuje s časom a statusom MCI a odráža buď úsilie o kompenzáciu, alebo maladaptívne zmeny, alebo kombináciu oboch procesov. Pokiaľ ide o motorickú reč u PD, poskytli sme náhľad na neurálne koreláty kontroly produkcie reči a reorganizáciu artikulačnej siete u PD zreteľnú už u pacientov bez subjektívnej poruchy reči. Exploratívna štúdia ukázala, že rTMS aplikovaná nad oblasť sluchovej spätnej väzby môže viesť k zlepšeniu reči u PD cez zlepšenie konektivity medzi oblasťami mozgu, ktoré sú zapojené v dorzálnej jazykovej dráhe.

Abstract:

Parkinson’s disease (PD) is a neurodegenerative disorder that affects millions of people worldwide and it is associated with variable involvement of core motor as well as non-motor symptoms. So far, there exists no therapeutic approach that would cure PD or permanently stop its progression. We focused both on structural and functional MRI changes in PD subtypes with a special interest in identifying neural correlates of the symptoms that are difficult to treat and have a major impact on quality of life, namely on cognitive impairment and hypokinetic dysarthria. The aim of this research was to identify specific structural and functional changes in PD patients related to distinct cognitive subtypes. Another objective was to assess neural correlates of hypokinetic dysarthria (HD), a frequent PD symptom, and to investigate effects of repetitive transcranial magnetic stimulation (rTMS) on HD. Firstly, we investigated brain structure and functional connectivity in PD patients using different methods. For exploring gray matter (GM) atrophy related to various cognitive PD subtypes, we applied voxel-based morphometry, source-based morphometry and cortical thickness analysis. When assessing functional connectivity changes in large-scale brain networks we used seed-based correlation analysis, partial least squares and graph theory network analysis in a prospective longitudinal study of PD patients and healthy age-matched controls (HC). In the second part of our work we examined neural correlates of speech control using evaluation of gray matter volumes in speech-related regions and functional connectivity of the relevant structures during a reading task in the MR scanner. In addition, we aimed at identifying optimal cortical sites for rTMS treatment in order to improve motor speech problems in PD patients. To achieve this goal, we explored short-term effects of repetitive transcranial magnetic stimulation (rTMS) applied over left orofacial primary motor area, the right posterior superior temporal gyrus (STG), and over the vertex (a control site) using high and low frequency rTMS. In the PD cognitive subtypes study, we found that PD with mild cognitive impairment (PD-MCI) showed greater GM atrophy than PD with normal cognition (PD-NC) in the orbitofrontal regions (voxel-based morphometry), which was related to memory z-scores, and in the left superior parietal lobule (cortical thickness); more widespread limbic and fronto-parieto-occipital neocortical atrophy (all methods) differentiated this group from HC. Only cortical thickness analysis revealed subtle GM atrophy in the anterior cingulate, precuneus, and temporal neocortex in PD-NC as compared to HC but these changes did not correlate with any cognitive outcomes. In the functional connectivity study, the between-network connectivity of the frontoparietal control network and other resting-state networks was reduced, and the whole-brain analysis revealed increased characteristic path length and decreased average node strength, clustering coefficient, and global efficiency in PD-NC compared to HC. Values of all measures in PD-MCI were between that of HC and PD-NC. After one year, the between-network connectivity was further increased in the PD-all group; no changes were detected in HC. In the motor speech study, PD patients as compared to HC displayed temporal decreases in speech loudness which were related to BOLD signal increases in the right-sided regions of the dorsal language pathway/ articulatory network. Among those regions, activation of the right anterior cingulate was increased in PD as compared to HC. We also found bilateral posterior superior temporal gyrus GM loss in PD as compared to HC that was strongly associated with diadochokinetic irregularity in the PD group. ffects were superior to the control site stimulation and were accompanied by increased resting state functional connectivity between the stimulated region and the right parahippocampal gyrus. In the PD cognitive subtypes study, voxel-based morphometry and cortical thickness analysis were more sensitive than source-based morphometry for identifying neural correlates of PD-MCI as compared to PD-NC and HC. Major differences were observed in the frontal limbic and superior parietal regions that are implicated in both memory and attention/executive function deficits that are characteristic for PD-MCI. Only cortical thickness measures revealed some subtle differences between HC and PD-NC and may thus have potential to serve as an early marker of PD. The functional connectivity study revealed that as compared to HC, PD-NC patients already display a less efficient transfer of information globally and reduced between-network connectivity particularly of the visual and frontoparietal control networks. The between-network connectivity increases with time and MCI status, reflecting either compensatory efforts or maladaptive changes, or combination of both processes. Regarding the motor speech in PD, we provided insight into the neural correlates of speech production control and distinct articulatory network reorganization in PD apparent already in patients without subjective speech impairment. The exploratory study shows that rTMS applied over the auditory feedback area may lead to improvement of speech in PD through enhanced connectivity among brain regions involved in the dorsal language pathway.
