Physiotherapy in a client with chronic lateral epicondylitis of the humerus

Abstract:

Název: Fyzioterapie u klienta s chronickou laterální epikondylitidou humeru. Cíl práce: Přispět k řešení problematiky fyzioterapie u klienta s chronickou laterální epikondylitidou humeru. Metodika: Teoretickou část tvoří rešerše nejčastěji používaných terapeutických postupů při fyzioterapii chronické laterální epikondylitidy. Hlavní částí je kazuistika fyzioterapie 49letého pacienta s laterální epikondylitidou v průběhu čtyř týdnů (dvě terapie týdně). Na základě vstupního kineziologického rozboru zahrnujícího parametry kvalitativního výzkumu podle Hendela, antropometrické vyšetření, měření hybnosti, testování kloubní vůle, svalového tonu, svalové testy, dynamické vyšetření páteře, vyšetření hlubokého stabilizačního systému a speciální testy byl sestaven krátkodobý fyzioterapeutický plán. V průběhu fyzioterapie byly využity konkrétní koncepty a metody - měkké techniky, zdravotní masáž, flossing, mobilizace kloubů, relaxační techniky (PIR, postfacilitační strečink), kompresivní terapie trigger pointů, centrace ramenního kloubu, excentrické posilování s činkami a cvičení s pomůckami Thera-Band, FlexBar, cvičení v uzavřených a otevřených kinematických řetězcích, PNF, DNS a ACT?. Po skončení fyzioterapie byl proveden výstupní kineziologický rozbor a navržen dlouhodobý fyzioterapeutický plán. Výsledky: Zlepšení rozsahu pohybů v ramenním kloubu PHK do zevní rotace o 10°, do vnitřní rotace o 20° a do horizontální addukce o 30°. Rozsah pohybu do pronace/supinace předloktí PHK se zvětšil o 10°. Zlepšila se svalová síla pravé lopatky ze stupně 4 na 5 ve všech testovaných směrech. Zmenšila se palpační a pozátěžová bolest postiženého lokte - pacient udává na numerické škále bolesti (NRS) subjektivní zlepšení z NRS = 8 na NRS = 5 a zvýšila se svalová síla při extenzi MP kloubů prstů ruky ze stupně 4 svalového testu podle Jandy na stupeň 5. Závěr: Konzervativní fyzioterapie chronické laterální epikondylitidy humeru přispěla ke zlepšení stavu pacienta - snížila se bolestivost lokte, byl zvýšen rozsah pohybu v ramenním kloubu a předloktí, došlo ke zvětšení svalové síly lopatky a svalové síly v oblasti MP kloubů do extenze.

Abstract:

Title: Physiotherapy in a client with chronic lateral epicondylitis of the humerus. The objective: Contribute to solving the problem of physiotherapy in a client with chronic lateral epicondylitis. Methodology: The theoretical part is processed as a research of the most frequently used therapeutic procedures in the physiotherapy of chronic lateral epicondylitis. The main part is a case study of physiotherapy of a 49-year-old patient with lateral epicondylitis during four weeks (two therapies per week). Based on the initial kinesiological analysis including parameters of qualitative research according to Hendel, anthropometric examination, measurement of momentum, joint play, muscle tone and tests, dynamic examination of the spine, examination of the deep spinal stabilization system and special tests, a short-term physiotherapy plan was drawn up. During physiotherapy specific concepts and methods were used - soft techniques, flossing, joint mobilization, relaxation techniques (PIR, postfacilitation stretching), compression therapy of trigger points, centering of the shoulder joint, eccentric strengthening with dumbbells and exercises with Thera-Band, FlexBar, exercises in closed and open kinematic chains, PNF, DNS and ACT?. Based on output kinesiological analysis was proposed a long-term physiotherapy plan. The results: Improvement of the range of motion in the shoulder joint to external rotation by 10°, to internal rotation by 20° and movement to horizontal adduction by 30°. Muscle strength of the right scapula improved from grade 4 to 5 in all directions tested. The palpation and post-exercise pain of the affected elbow decreased - the patient reported a subjective improvement from NRS = 8 to NRS = 5 on the numerical pain scale (NRS) and the muscle strength during extension of the MP joints of the fingers of the hand increased from grade 4 of the Janda muscle test to grade 5. Conclusion: Conservative physiotherapy of chronic lateral epicondylitis of the humerus contributed to the improvement of the patient's condition - the pain of the elbow decreased, the range of motion in the shoulder joint and forearm increased, the muscle strength of the scapula and muscle strength in the area of the MP joints increased to extension.
