Interakce elektromagnetického pole s kardiostimulační technikou a jeho vliv na interpretaci intrakardiálních signálů

Abstract:

Cílem disertační práce je analýza chování a odezvy implantabilního kardiostimulačního zařízení v přítomnosti vnějšího zdroje elektromagnetické interference. Zaměřuji se na přístroje pro bradyarytmickou léčbu, tedy kardiostimulátory. Práce popisuje úvod do problematiky elektrofyziologie srdce, vymezuje pojmy a definuje základní vztahy z teorie elektromagnetismu. Poukazuji na význam programace kardiostimulačních systémů. Stimulátor uvažuji jako aktivní implantát umístěný v hrudníku pacienta. Chybná interpretace interferenčních signálů jako vlastní elektrická srdeční aktivita může narušit správnou funkci stimulátoru. Stěžejní je přechod vodič - tkáň u běžných intrakardiálních bipolárních elektrod. Detekci rušení ovlivňují funkční principy kardiostimulátoru ve smyslu jeho programování a časování. Diskutovány jsou především programové možnosti moderních systémů pro minimalizaci rizika pro pacienta. To popisuji na klinických epizodách, které porovnávám se simulovanými záznamy z prováděných experimentů. Zaměřuji se na možnosti in vitro testování na fantomu kardiostimulátoru v rizikovém elektromagnetickém prostředí. Dále diskutuji možnosti pro využití povrchového elektrokardiogramu k posouzení interakce. Na závěr navrhuji řešení pro predikci důsledků interakce formou modulárního monitorovacího systému blízkých elektromagnetických polí.

Abstract:

The aim of the dissertation thesis is to analyse the behaviour and the response of the cardiac implantable electronic device in the presence of an external source of electromagnetic interference. I focus on devices for bradyarrhythmia treatment, i.e. pacemakers. The thesis describes the introduction to the electrophysiology of the heart, defines terms and basic relations from the theory of electromagnetism. I point out the importance of programming cardiac pacing systems. The device is considered as an active implant placed in the patient's chest. The improper interpretation of the disturbing signals as intrinsic electrical cardiac activity can affect the proper function of the device. Crucial is the lead-tissue transition using conventional intracardiac bipolar electrodes with an active fixation. The detection of interference is affected by the functional principles of the pacemaker, its programming and timing. The software possibilities of modern systems for minimizing the risk for the patient are discussed. I describe this issue on clinical episodes, which are compared with demonstrated events from realized experiments. I focus on the possibilities of in vitro testing on the pacemaker phantom in a risky electromagnetic environment. The limitations of using surface electrocardiogram to assess the interaction are discussed. Finally, I propose a solution for predicting the consequences of the interaction in the form of the modular monitoring system of near electromagnetic fields.
