Detection of HER2/neu in breast cancer

Abstract:

Tato práce se zabývá detekcí prognostického a prediktivního markeru HER2/neu u karcinomu prsu. Nejprve je vymezena charakteristika onkoproteinu HER2/neu. Dále je popsán algoritmus vyšetřování a hodnotící kritéria tohoto markeru. Současně jsou zde uvedena možná úskalí jednotlivých kroků hodnocení exprese markeru. Práce obsahuje teoretický základ standardně používaných metod pro vyšetřování exprese onkoproteinu HER2/neu. Uváděné metody jsou podpořeny praktickým popisem imunohistochemického a hybridizačního vyšetření užívaného v histologické periferní laboratoři v Pardubické krajské nemocnici a.s. a v histologické referenční laboratoři Fingerlandova ústavu patologie Fakultní nemocnice v Hradci Králové. Součástí práce je statistické zpracování dat pacientek s karcinomem prsu. Tato data byla získána z histopatologických vyšetření v laboratoři Pardubické krajské nemocnice a.s. Zahrnují časové rozpětí od 1. ledna 2009 do 30. dubna 2011. Zde je sledována jako prvotní hledisko závislost onkoproteinu HER2/neu na vybraných faktorech imunohistochemických vyšetření. Práce je doplněna informacemi o léčbě HER2-pozitivních karcinomů. Zde jsou uvedena kritéria vedoucí k nalezení vhodné skupiny pacientek pro nasazení biologické léčby. Výsledky této terapie jsou v příloze doloženy daty získanými z projektu Herceptin. V příloze je práce obohacena porovnáním výsledků statistického zpracování dat z Pardubické krajské nemocnice a.s. za uvedené období s výsledky podobné studie provedené za rok 2009 ? 2010 Ústavem pro péči o matku a dítě v Praze.

Abstract:

This thesis deals with detection of prognostic and predictive marker HER2/neu relevant to breast cancer. The characteristic of oncoprotein HER2/neu is defined first of all. Further the algorithm of investigation and evaluative criteria of this marker are included. Certain possible problems of each step of evaluation concerning expression of this marker are natural part of this project. My thesis also includes the theoretical basis of the standard methods which are used for the investigation of expression of oncoprotein HER2/neu. These methods are supported by a practical description of hybridization and immunohistochemical examinations used in peripheral histological laboratory in Regional Hospital Pardubice and also by the reference histological laboratory in Fingerland´s Institute of Pathology, Univesity College Hospital Hradec Králové. At the same time this thesis contains statistical processing of data of patiens with breast cancer. These data originate from histopathological examinations in a laboratory in Regional Hospital Pardubice in a span from 1.1.2009 to 30.4.2011 and monitor the dependency of oncoprotein HER2/neu in connection with selected factors of immunohistochemical examinations. This thesis provides information on the treatment of HER2-positive carcinomas and includes the criteria for finding a suitable group of patiens for the use of biological treatment. The results of this therapy which were received during the project Herceptin are in the attachment. The thesis is enriched by the attachment where there is a comparison between the statistical data processing concerning a foregoing span from Regional Hospital Pardubice and the results of similiar study executed in 2011 by Institute for Care of Mother and Child in Prague.
