The effectiveness of rehabilitation in patients after stroke aspect of a comprehensive clinical testing

Abstract:

Úvod: Diplomová práce předkládá přehled ověřených poznatků o využití klinickéhotestování pro hodnocení efektivity rehabilitace u pacientů po cévní mozkové příhodě a doplňuje je o závěry vlastní studie. Zaměřuje se na hodnocení motorických, senzitivních a sesnsorických změn dolní končetiny v průběhu terapie hemiparetiků. Blíže specifikuje využití a náležitosti klinického testu Fugl-Meyer Assessment a chůzového testu 10 Metre Walk Test. Cíl: Pomocí klinického testování posoudit efektivitu rehabilitace u pacientů v subakutním stádiu po cévní mozkové příhodě, se zaměřením na dolní končetinu. Metodika: Studie se zúčastnilo 16 probandů v subakutní fázi po cévní mozkové příhodě. Všichni probandi podstoupili každý všední den dvě 45 minutové terapie, zaměřené na obnovu funkčnosti končetin a trupu, nácvik stereotypu chůze a nácvik ADL aktivit. Ke zhodnocení efektivity rehabilitace byly použity testy Fugl-Meyer Assessment a 10 Metre Walk Test. Výsledky: V testu Fugl-Meyer Assessment došlo ke statisticky významnému zlepšení parametru čití (p = 0,021), u parametru taxe bylo dosaženo hraniční hodnoty statistické významnosti (p = 0,053), je tedy možné sledovat trend zlepšení parametru. Pomocí 10 Metre Walek Test bylo zjištěno statisticky významnému zvýšení rychlosti chůze (p = 0,001). Závěr: Oba testy navrhuje jako vhodnou formu testování v klinické praxi fyzioterapeuta.

Abstract:

Introduction: The diploma thesis presents the overview of verified findings about the usage of clinical trials in order to assess the rehabilitation effectiveness among post-stroke patients while it adds conclusions of the study itself. It is focused on the assessment of motor, sensitive and sensor changes of the lower limb in the course of hemiparetic therapy. It further specifies the employment and requisites of Fugl-Meyer Assessment and 10 Metre Walk Test. Aim: Via clinic trials, to assess the rehabilitation effectiveness among post-stroke patients in sub-acute stage, focusing on the lower limb. Methods: Sixteen probands after a stroke in sub-acute stage participated in the study. Every weekday, all of them underwent two 45-minute-long therapy sessions which were focused on the restoration of limbs' and trunk functionality, training of the walk stereotype and training of ADL activities. In order to assess the rehabilitation effectiveness, the tests Fugl-Meyer Assessment and 10 Metre Walk Test were used. Results: In the Fugl-Meyer Assessment, the parameter of perception was statistically significantly improved (p = 0,021), the parameter of taxis reached the threshold value of statistical significance (p = 0,053), therefore, it is possible to follow the improving trend of the parameter. Via the 10 Metre Walk Test, a statistically significant improvement (p = 0,001) of the walking speed was discovered. Conclusion: Both tests are suggested to be a suitable form of assessment in the clinical practice of a physiotherapist.
