The effect of regular respiratory physiotherapy on long-term mechanical ventilated patients

Abstract:

Úvod: Cílem této diplomové práce bylo zjistit vliv pravidelně prováděných technik respirační fyzioterapie na vybrané respirační parametry pacientů dlouhodobě připojených k umělé plicní ventilaci. Vybráni byli pacienti, u kterých selhal weaning a jejich šance na odpojení od ventilátoru jsou nízké. Jednalo se o pasivně prováděné techniky jako techniky měkkých tkání, kontaktní dýchání, vibrace a reflexní dýchání dle Vojty. V teoretické části je pospána mechanika respiračního systému, principy umělé plicní ventilace, monitorování klinického stavu pacientů a vybrané techniky respirační fyzioterapie. Metodika: Výzkum probíhal na oddělení dlouhodobé intenzivní péče Nemocnice AGEL v Přerově. Byli vybrání probandi splňující tato kritéria: režim bifázické nebo kontinuální ventilace pozitivním přetlakem, intubováni tracheostomií s primární diagnózou akutního respiračního selhání různé etiologie, minimální doby ventilace před zahájením terapie nejméně jednoho měsíce, selhání weaningu. Tato kritéria v době měření splnilo celkem 7 pacientů oddělení. Metody respirační fyzioterapie byly u pacientů prováděny jednou denně s preferencí v dopoledních hodinách po celkovou dobu 28 dní, včetně víkendů. Respirační parametry byly odečítány z přístroje umělé plicní ventilace a monitoringu životních funkcí. Výsledky: Měřené parametry byly statisticky vyhodnoceny a zpracovány do grafů a tabulek. Výsledky neprokázaly statisticky významný vliv respirační fyzioterapie na ventilační parametry dlouhodobě ventilovaných pacientů vyjma saturace, kde došlo k jejímu navýšení bezprostředně po terapii. Závěr: Na základě výsledků práce nebyl potvrzen vliv respirační fyzioterapie na dlouhodobě uměle ventilované pacienty po bezprostředním ukončení terapie, ani v dlouhodobém horizontu 28denní intervence.

Abstract:

Background: The aim of this diploma thesis was to determine the influence of regularly performed respiratory physiotherapy techniques on selected respiratory parameters of patients connected to mechanical ventilation for a long time. Patients, in whom weaning failed and their chances of disconnection from the ventilator are low, were selected. These were passively performed techniques such as soft tissue mobilization, contact and resistance breathing, vibration and reflex breathing according to Vojta´s method. The theoretical part describes the mechanics of the respiratory system, the principles of mechanical ventilation, monitoring the clinical condition of patients and selected techniques of respiratory physiotherapy. Methodology: The research took place at the long-term intensive care department of the AGEL Hospital in Přerov. Probands meeting the following criteria were selected: bilevel or continuous positive airway pressure ventilation, intubated with tracheostomy with a primary diagnosis of acute respiratory failure of various etiologies, minimum ventilation time before starting therapy of at least one month, weaning failure. A total of 7 patients in the ward met these criteria at the time of the measurement. Respiratory physiotherapy methods were performed once per day within patients in the morning for a total of 28 days, including weekends. Respiratory parameters were read from the device of mechanical ventilation and from the vital signs monitoring. Results: The measured parameters were statistically evaluated and processed into graphs and tables. The results did not show a statistically significant effect of respiratory physiotherapy on the ventilation parameters of long-term ventilated patients, except for saturation, where it increased immediately after therapy. Conclusion: Based on the results of the work, it was not possible to confirm the effect of respiratory physiotherapy on long-term mechanical ventilated patients after the immediate termination of therapy, or in the long-term horizon of 28-day intervention.
