Use of flow cytometry in the diagnosis of heparin-induced thrombocytopenia

Abstract:

Úvod: Heparinem indukovaná trombocytopenie je klinicko-patologický stav, který může nastat jako komplikace při léčbě heparinem, častokrát společně s komplikací trombózy. Diagnostika heparinem indukované trombocytopenie se skládá z klinické a laboratorní části. Základem klinické části je 4T´s skóre, podle kterého se odvíjí laboratorní diagnostika. Cíl: Zjistit využití průtokové cytometrie při diagnostice heparinem indukované trombocytopenie pomocí srovnání naměřených výsledků s výsledky dalších diagnostických testů za současného využití základních statistických metod. Vyšetřovaný soubor obsahoval celkem 29 pacientů, z toho 20 můžu a 9 žen. Průměrný věk pacientů byl 67 let. Výsledky: Na základě pozitivních výsledků screeningového stanovení protilátek proti komplexu destičkového faktoru 4 s heparinem celého vyšetřovaného souboru pacientů se suspektní heparinem indukovanou trombocytopenií se provedly vyšetření ELISA HIT testů, světelné transmisní agregometrie a průtokové cytometrie. K vyšetření protilátek byly použity testy DiaMed, STic Expert HIT a senzitivnější ELISA test. Agregace byla vyšetřena na přístroji APACT 4004 a k cytometrii byl použit průtokový cytometr Navios Ex. Výsledky 3 metod se mezi sebou porovnaly, aby se zjistila možná shoda mezi metodami. Ke statistickému srovnání byl použit statistický software MedCalc, který vypočítal určité statistické veličiny.

Abstract:

Introduction: Heparin-induced thrombocytopenia is a clinical-pathological condition that can occur as a complication of heparin treatment, often together with the complication of thrombosis. The diagnosis of heparin-induced thrombocytopenia consists of a clinical and a laboratory part. The basis of the clinical part is the 4T's score, according to which the laboratory diagnosis is developed. Objective: To find out the use of flow cytometry in the diagnosis of heparin-induced thrombocytopenia by comparing the measured results with the results of other diagnostic tests while using basic statistical methods. The examined group contained a total of 29 patients, of which 20 were men and 9 were women. The average age of patients was 67 years. Results: Based on the positive screening results of antibodies against the complex of platelet factor 4 with heparin of the entire investigated group of patients with suspected heparin-induced thrombocytopenia, ELISA HIT tests, light transmission aggregometry and flow cytometry were performed. DiaMed, STic Expert HIT and a more sensitive ELISA test were used to examine antibodies. Aggregation was assayed on an APACT 4004 instrument and flow cytometry was performed using a Navios Ex flow cytometer. The results of the three methods were compared with each other to determine possible agreement between the methods. For statistical comparison, the MedCalc statistical software was used, which calculated certain statistical quantities.
