Analysis of chromosomal aberrations in uveal melanoma

Abstract:

Maligní melanom cévnatky je jedním ze třech typů uveálního melanomu, který vzniká z melanocytů umístěných v cévnatce. Jedná se o nejčastější primární zhoubný nádor oka v dospělém věku, jehož incidence je 2 – 8 případů na jeden milion obyvatel. Jde tedy o poměrně vzácné onemocnění. Současným trendem u melanomu je sledování genetické povahy nádoru, ať už se jedná o mutace v iniciačních genech či o sledování nenáhodných chromozomových změn. Chromozomové abnormality jsou podstatným prognostickým markerem a spolu s dalšími prognostickými proměnnými (klinické a histopatologické faktory) se podílejí na odhadu chování nádorového onemocnění. Monozomie chromozomu 3 (M3), zisk dlouhého raménka chromozomu 8 (8q+) a zisk krátkého raménka chromozomu 6 (6p+) jsou častými změnami a mohou se spolu různě kombinovat. Tyto abnormality jsou spojovány s progresí nádorového onemocnění a s rozvojem metastáz. V doposud zavedené praxi se tyto změny chromozomů stanovují pomocí in situ hybridizace. Tato diplomová práce se zaměřuje na stanovení výše zmíněných chromozomálních abnormalit pomocí chromogenní a fluorescenční in situ hybridizace. Nejprve je popsán soubor pacientů a potřebný materiál k realizaci praktické části. Následuje popis zpracování oční tkáně, dále popis samotných ISH metodik a jejich optimalizace. Vyhodnocení výsledků ISH je provedeno pomocí tabulek, obrázků a grafů.

Abstract:

Choroidal malignant melanoma is one of three types of uveal melanoma that arises from melanocytes located in the choroid. It is the most common primary malignancy of the eye in adulthood, with an incidence of 2-8 cases per million inhabitants. It is therefore a relatively rare disease. The current trend in choroval melanoma is to monitor the genetic nature of the tumor, whether they are mutations in the initiation genes or a monitoring of non-random chromosomal changes. Chromosomal abnormalities are an essential prognostic marker and together with other prognostic variables (clinical and histopathological factors) involved in estimating cancer behavior. The monosomy of chromosome 3 (M3), the gain of the long arm of chromosome 8 (8q +) and the gain of the short arm of chromosome 6 (6p +) are frequent changes and can be combined together in various ways. These abnormalities are associated with cancer progression and the development of metastases. In current practice, these chromosome changes are determined by in situ hybridization. This diploma thesis focuses on the determination of the above mentioned chromosomal abnormalities by chromogenic and fluorescent in situ hybridization. First, a cohort of patients and the necessary material for the implementation of the practical part are described. The following is a description of the processing of eye tissue, as well as a description of the ISH methods themselves and their optimalization. The evaluation of ISH results is performed using tables, figures and graphs.
