Patient Compromited by Haemodynamic Instability - from Prehospital Care through Emergency to Intensive Care Unit

Abstract:

Bakalářská práce se zabývá problematikou hemodynamiky a šokovými stavy jakožto její hlavní poruchou. Cílem práce je poukázat na to, že ne všechny šokové stavy mají stejné terapeutické intervence a u každého typu šoku může klinická manifestace probíhat jinak. Někdy ale i přes veškerou snahu lékařského i nelékařského zdravotnického personálu, správným intervencím, rychlému transportu a následnému zajištění pacienta v akutní nemocniční péči dochází k progresi stavu, který už může být ireverzibilní. Teoretická část práce se zaobírá vymezení pojmu hemodynamika, její součástí a jaké jsou možnosti monitorace. Další součástí jsou šokové stavy, jejich rozdělení, patofyziologie a možné intervence při řešení konkrétního typu šoku. V praktické části jsou zpracovány kazuistiky oběhově nestabilních pacientů v šokovém stavu. Cílem této části práce je popsat postup pacienta od přednemocniční péče přes transport a příjem na vysokoprahový urgentní příjem a následně na anesteziologicko – resuscitační oddělení. Jde tedy o primární kontakt s pacientem, diagnostiku a intervence, které byly provedeny, a následně po základní neodkladné stabilizaci umístění pacienta na finální pracoviště akutní lůžkové péče.

Abstract:

The bachelor thesis deals with the issue of hemodynamic and shock states as its main disorder. The aim of this work is to point out that not all shock conditions have the same therapeutic interventions and for each type of shock the clinical manifestation may be different. Despite of medical staff efforts, proper intervention, immediate transport and subsequent securing patient in emergency hospital treatment, patient`s condition could get worse even can be irreversible. In theoretical part, this thesis deals with the definition of the concept of hemodynamic, its components and possibilities of monitoring. Other components include shock conditions, their classification pathophysiology, and possible interventions in dealing with specific type of shock. In practical part, case reports of circulatory unstable patients in shock are processed. Aim of this part of the work is to describe patient`s procedure from prehospital care through transport and admission to emergency and after all to intensive care unit. Therefore, it is primary contact with patient, diagnosis and intervention that were performed and after urgent stabilization allocation of patient in the final place of intensive care unit.
