Pre-hospital Care of a Patient with Dyspnoea from the Perspective of a Paramedic

Anotace:

Tématem bakalářské práce je dušnost v přednemocniční péči. Cíle práce jsou rozděleny na cíle teoretické části a cíle praktické části. Teoretická práce se zabývá problematikou dušnosti, vznikem a diferenciální diagnostikou dále popisuje farmakologickou terapii dušnosti v přednemocniční péči. V teoretické části je popsána anatomie dýchacích cest, dále je zde popsána diferenciální diagnostika dušnosti, jsou vybrány nejčastější typy dušnosti, rozděleny jsou na kardiální a pulmonální. Dále jsou zde popsány vyšetřovací metody v přednemocniční péči, popsány jsou jak fyzikální vyšetření tak vyšetření za pomoci přístrojů. Je zde popsán i algoritmus cABCDE a jeho využití. V praktické části jsou popsány vybrané kazuistiky a stanovení doporučení pro praxi zdravotnického záchranáře při ošetření pacienta s dušností. V praktické části práce jsou popsány čtyři kazuistiky, ke každé kazuistice je vedena diskuze, která posuzuje daný výjezd. V závěru praktické části je diskuze, která porovnává postup v uvedených kazuistikách a postup uvedený v literatuře. Z kazuistik a z diskuze vychází řada doporučení pro praxi zdravotnického záchranáře v přednemocniční neodkladné péči.

Abstract:

The topic of the bachelor thesis is dyspnoea in pre-hospital care. The aims of the thesis are divided into the aims of the theoretical part and the aims of the practical part. The theoretical thesis deals with the problem of dyspnoea, its origin and differential diagnosis, and describes the pharmacological therapy of dyspnoea in pre-hospital care. In the theoretical part the anatomy of the airways is described, the differential diagnosis of dyspnoea is described, the most common types of dyspnoea are selected, they are divided into cardiac and pulmonary. Furthermore, the examination methods in pre-hospital care are described, both physical and instrumental examinations are described. The cABCDE algorithm and its use is also described. In the practical section, selected case reports are described and recommendations for paramedic practice in the treatment of the patient with dyspnoea are established. In the practical part of the thesis, four case reports are described, and a discussion is given for each case report that assesses that particular call. The practical part concludes with a discussion that compares the procedure in these case studies and the procedure reported in the literature. A number of recommendations for paramedic practice in pre-hospital emergency care emerge from the case studies and the discussion.
