Identification of gram-positive rods isolated from clinical material by MALDI-TOF mass spectrometry method

Abstract:

Hmotnostní spektrometrie MALDI-TOF (laserová desorpce a ionizace za účasti matrice s průletovým analyzátorem) je stále častěji užívanou metodou pro identifikaci širokého spektra mikroorganismů v klinických laboratořích. MALDI-TOF tak díky své rychlosti a přesnosti identifikace nahradila klasické biochemické metody, které v případě grampozitivních tyček vykazují nedostatečné výsledky. Bakalářská práce obsahuje charakteristiku identifikovaných kmenů grampozitivních tyček, jejich výskyt a klinický význam. Dále je zde popsán princip hmotnostního spektrometru MALDI-TOF a způsoby přípravy materiálu k hmotnostní analýze. Cílem této práce je identifikovat 45 kmenů grampozitivních tyček zpracovávaných třemi způsoby - metoda bez extrakce, rychlá extrakce 70% kyselinou mravenčí a úplná (komplexní) extrakce 70% kyselinou mravenčí a zhodnotit jejich dopad na úspěšnost identifikace. Dle získaných výsledků lze potvrdit, že i přes časovou náročnost komplexní extrakce, ji doporučujeme provádět u klinicky významných, avšak těžce identifikovatelných, druhů.

Abstract:

MALDI-TOF (matrix-assisted laser desorption ionization - time of flight) mass spectrometry is a method increasingly used for the identification of a wide range of microorganisms in clinical laboratories. Owing to its rapid and accurate identification, MALDI-TOF has replaced classical biochemical methods, which provide inaccurate results when applied to gram-positive rods. This Bachelor thesis outlines the characteristics of identified strains of gram-positive rods, their occurrence and clinical significance. The principle of the MALDI-TOF Mass Spectrometer and methods for the preparation of material for the mass analysis are also described. The aim of this thesis is to identify 45 strains of gram-positive rods prepared using three methods (method without extraction, fast 70% formic acid extraction, complex 70% formic acid extraction) and evaluate how these methods influence the success of identification. Based on the results obtained, it can be confirmed that despite its time-consuming nature, we recommend that the complex extraction be used for species that are difficult to identify but clinically important.
