The evolution of the rehabilitation process from the acute phase to the return to the home of an adult patient suffering from Guillain-Barré Syndrome: a clinical case

Abstract:

Guillain-Barré syndrome is a disease that affects 1.1 to 1.8 people per 100,000 per year, resulting in an important disease in terms of managing the entire process that the patient has to complete. It is in fact defined as an acute idiopathic inflammatory demyelinating polyradiculopathy, that is a pathology that starts in most cases from an acute infectious event given by some bacteria that carry, given their similarity of membrane with some receptors at the level of the myelin sheath, to an immune response to the Schwann cells leading them to death. This results in an acute inflammation that can be localized to more peripheral nerves being a systemic disease. This essential characteristic therefore leads to an important variation between the various patients, with cases of injury to all the peripheral nerves thus leading to the need for ventilation support for patients who show very small symptoms and signs. From this, the methods of intervention tend to be the same in the acute phase, with an evolution of these that may differ depending on the extent of the inflammatory radiculopathy. For this reason it is important to identify and prepare the patient for the subsequent recovery phases in the best possible way. In fact, this pathology almost always has a very positive prognosis; unfortunately this is not always true if we talk about the numerous variants that there may be, where in most cases there is also a lesion of the axon and not only of the myelin sheath that covers them. The clinical case presented describes the return to a normal life of a patient with a particular recovery presentation as it goes against the standard. The choices made both by the hospital team and subsequently in the final rehabilitation field were focused on prevention and total recovery of the patient, having full support and collaboration from the latter. During the recovery phase it was necessary to take into account many variables and risks related to the patient's inability to voluntarily and involuntarily feel and move the lower limbs, causing the physiotherapist to evaluate the exercises very carefully and in general the rehabilitation of the patient.

La sindrome di Guillain-Barré ? una patologia che colpisce da 1,1 a 1,8 di soggetti ogni 100.000 all'anno, risultando una malattia importante per quanto riguarda la gestione di tutto l'iter che il paziente deve compiere. Essa infatti ? definita come una poliradicolopatia demielinizzante infiammatoria acuta idiopatica, ovvero una patologia che parte nella maggior parte dei casi da un evento acuto infettivo dato da alcuni batteri che portano, vista la loro somiglianza di membrana con alcuni recettori posti a livello della guina mielinica, porta ad una risposta immunitaria verso appunto le cellule di Schwann portandole alla morte. Questo si traduce in una infiammazione acuta che pu? essere localizzata a pi? nervi periferici essendo una malattia comunque sistemica. Questa essenziale caratteristica porta quindi ad una variazione importate tra i vari pazienti, con casi di lesione a tutti i nervi periferici portando quindi al bisogno di un supporto per la ventilazione a pazienti che manifestano sintomi e segni molto piccoli. Da questo le modalit? di intervento sono tendenzialmente le medesime in fase acuta, con un'evoluzione di questi che possono differire a seconda dell'estensione della radicolopatia infiammatoria. Per questo ? importante individuare e preparare il paziente il paziente alle fasi di recupero successive nel migliore dei modi. Infatti questa patologia presenta quasi sempre una prognosi molto positiva; questo purtroppo non ? sempre vero se si parla delle numerose varianti che ci possono essere, dove nella maggior parte dei casi vi ? una lesione anche dell'assone e non solo della guina mielinica che le riveste. Il caso clinico presentato descrive il ritorno ad una vita normale di una paziente con una particolare presentazione di recupero in quanto va contro allo standard. Le scelte fatte sia dall'equipe ospedaliera che successivamente in ambito riabilitativo finale sono state incentrate alla prevenzione e al recupero totale della paziente, avendo da parte di quest'ultima un pieno appoggio e collaborazione. Durante la fase di recupero si ? dovuto tener conto di molte variabili e di rischi connessi all'impossibilit? della paziente di sentire e muovere volontariamente e involontariamente gli arti inferiori, facendo si che il fisioterapista abbia dovuto valutare in maniera molto attenta gli esercizi e in generale il percorso riabilitativo della paziente.
