Possibilities, limits and problems with rehabilitation by patients with complicated fractures of a lower limb dealed with osteosynthesis of external immobilizers

Abstract:

Zevní fixátory jsou implantáty, které stabilizují zlomeninu perkutánně zavedenými hřeby (šrouby, dráty). Tyto hřeby jsou zevně, mimo tělo pacienta, spojeny úhlově stabilními fixačními elementy. Jejich výhodou je možnost širokého využití, od stabilizace běžných fraktur, až po léčení vrozených deformit. Nejčastější indikací užití jsou komplikované otevřené zlomeniny nebo fraktury spojené s rozsáhlým poškozením měkkých tkání. Zevní fixátory se mohou dělit podle způsobu aplikace, ukotvení v kosti nebo užitých konstrukčních prvků. K ukotvení v kosti lze použít Schanzovy šrouby, Steinmannovy hřeby, zejména u jednorovinné aplikace, nebo Kirschnerovy dráty v kruhových kompresně ? distrakčních aparátů. Jednorovinné fixátory méně omezují kloubní pohyblivost a zmírňují pacientův dyskomfort. Kombinací obou těchto způsobů aplikace (jednorovinné, kruhové) vznikají tzv. hybridní fixátory. Rehabilitace u všech typů zevních fixátorů je podobná rehabilitaci při konzervativní terapii. V období naložení zevního fixátoru je snaha udržet rozsah pohybu v nefixovaných kloubech a udržet svalovou sílu. K vertikalizaci pacienta lze přistoupit po zmírnění bolesti, to je již 2 ? 3 den po operaci. Zpočátku se nacvičuje chůze s plným odlehčením postižené dolní končetiny a podle stupně hojení kosti se postupně dovoluje zvětšování zatížení. Cílem rehabilitace je dosažení co nejlepšího stavu nemocného, tak aby jeho návrat k plnohodnotnému životu byl co nejrychlejší

Abstract:

The external immobilizers are implants, which stabilize a fracture by percutaneously inserted nails (screws, wires). These nails are externally, out of the patient?s body, jointed with stable fixation elements. Their advantage is a possibility of a wide scope use from a stabilization of common fractures to a treatment of congenital deformities. The most common use of this indication is with complicated compound fractures or fractures connected with an extend injury of soft tissues. The external immobilizers can be divided according to the method of administration, the anchor in a bone or according to used contrastive elements. Shanz?s screws or Steinmann?s nails can be particularly used to anchor the bone by one-plane administration. Or we might use the Kirschner?s wires by rounded distractional compression apparatus. The one-plane immobilizers limit the joints? mobility less and allay the patient?s discomfort. By the combination of both of the administration methods (one-plane, rounded), so called hybrid immobilizers come up. The rehabilitation in each type of external immobilizers is quite similar to the conservative therapy. During the period of anchoring the external immobilizer, the endeavour is to keep the scope of mobility in the unimmobilized joints and to keep the muscle strength. The verticalization of the patient might be provided after an alleviation of pain, which means two or three days after the surgery. At the beginning a walk on the handicapped lower limb is exercised, with a full load relief and according to the level of pain the load is then slowly increased. The aim of the rehabilitation is to reach the best possible state of the patient, to return him/her back to full life as soon as possible
