Life expectancy of patients with selected diagnoses

Abstract:

Bakalářská práce se zabývá očekávanou dobou dožití u pacientů s vybranými onemocněními. Očekávaná doba dožití je běžně používanou populační demografickou charakteristikou, z hlediska interpretace je nicméně zajímavá i pro analýzu na úrovni pacientů s vybranými diagnózami, jakožto nástroj pro vyhodnocení úspěšnosti léčby daného onemocnění na populační úrovni. Cílem práce je nastudovat výpočet očekávané doby dožití, její limitace a podmínky použití a aplikovat jej na podkladová data pacientů čtyř vybraných diagnóz, které byly vybrány, aby reprezentovaly šíři možných scénářů pro tento druh výpočtu. První část práce se zabývá očekávanou dobou dožití jak na populační úrovni, tak na úrovni konkrétních diagnóz, poskytuje rovněž krátký přehled informací o každé z vybraných diagnóz. Druhá část práce je věnovaná úmrtnostním tabulkám a metodice výpočtu očekávané doby dožití. Praktická část prezentuje získané výsledky analýzy, soustředí se rovněž na zohlednění specifik různých diagnóz a aplikaci adekvátní metodiky. U onkologických diagnóz přistupujeme k výpočtu incidenčním způsobem a očekávanou dobu dožití počítáme k věku při diagnóze, u zbylých dvou onemocnění, roztroušené sklerózy a diabetu, jsme omezeni dostupným časovým oknem a volíme prevalenční způsob výpočtu. V průběhu let lze spatřit prodlužování očekávané doby dožití jak u pacientů, tak u obecné populace. Rovněž si lze všimnout, že ženy mají obecně vyšší očekávanou dobu dožití než muži a rozdíly v očekávané době dožití mezi pacienty a obecnou populací se s rostoucím věkem snižují.

Abstract:

The bachelor thesis deals with the life expectancy of patients with selected diseases. Life expectancy is a commonly used population demographic characteristic, however, it is also of interest for interpretation at the level of patients with selected diagnoses, as a tool for evaluating the success of treatment of a given disease at the population level. The aim of this thesis is to study the calculation of life expectancy, its limitations and conditions of use and to apply it to the data of patients with four selected diagnoses, chosen to represent the breadth of possible scenarios for this kind of calculation. The first part of the thesis deals with life expectancy both at the population level and at the level of specific diagnoses, and also provides a brief overview of information on each of the selected diagnoses. The second part of the thesis is devoted to mortality tables and the methodology for calculating life expectancy. The practical part presents the results of the analysis, focusing also on taking into account the specifics of different diagnoses and the application of an adequate methodology. For cancer diagnoses, we approach the calculation in an incidence-based manner and calculate life expectancy according to age at diagnosis, for the other two diseases, multiple sclerosis and diabetes, we are limited by the available time window and choose a prevalence-based calculation. Over the years we can see an increase in the life expectancy of both patients and the general population. It is also noticeable that women generally have a higher life expectancy than men and the differences in life expectancy between patients and the general population decrease with increasing age.
