Bio-psycho-social needs of children with chronic pain

Abstract:

Cíl: Cílem diplomové práce bylo popsat a zjistit, jaké jsou bio-psycho-sociální potřeby dětí s chronickou bolestí. Dále pak zjistit, jakým způsobem děti vnímají chronickou bolest, obecně i ve vztahu k sobě samým, jaký význam jí připisují a jak bolest ovlivňuje jejich život. Metodika: Pro výzkum byla zvolena metoda kvalitativní. Výzkumný soubor tvořilo 6 dětí s chronickou bolestí (4 dívky a 2 chlapci), z toho 3 dívky a 1 chlapec z dětské revmatologické ambulance, 1 dívka hospitalizovaná na oddělení větších dětí FNO a 1 chlapec hospitalizovaný v Hospici sv. Lukáše. Pro zjištění potřebných informací byl použit strukturovaný nestandardizovaný rozhovor s rodičem a dítětem a pozorování. Sekundárním zdrojem informací byla zdravotnická dokumentace a lékařské zprávy. Informace byly se souhlasem participantů zaznamenány na diktafon a přepsány do elektronické podoby. Data byla uspořádána do tematických kategorií a subkategorií, které se týkaly bio-psycho-sociálních potřeb dítěte s bolestí. Výsledky: Šetřením bylo zjištěno, že chronická bolest dětem i jejich rodinám velmi komplikuje život. Z výsledků výzkumu vyplývá, že to, co děti a rodiče nejvíce potřebují, je především fungující rodina, dobré mezilidské vztahy, adekvátní informovanost, opravdový zájem zdravotnického personálu a holistický přístup k dětskému pacientovi. Jejich největším přáním je, aby se uzdravili, neměli chronické bolesti kloubů a nemuseli užívat a aplikovat léky a cytostatika. Velkou potřebou je pro všechny stav bez bolesti, zdraví a plnohodnotný život, jaký byl dříve, než propuklo chronické onemocnění a chronické bolesti. Závěr: V závěru práce lze shrnout, že dlouhodobá, chronická bolest dítě velmi ovlivňuje. U dětí je často spojena s mnoha nepříjemnými zážitky strachu, úzkosti a strádáním dítěte na bio-psycho-sociální úrovni. Je pro děti i jejich rodiny obrovskou zátěží a negativním způsobem ovlivňuje jejich život. Nejvíce to bylo znát v oblasti tělesné, školní a emocionální.

Abstract:

Aim: The aim of the diploma thesis was to discover and describe the psycho-biological and social needs of children with chronic pain. Further objectives were to determine how children perceive chronic pain, both generally and in relation to themselves, and how pain affects their lives and what significance do they ascribe to it. Methods: The chosen method was a qualitative research and the research group was formed by six children with chronic pain (four girls and two boys). Three girls and one boy were from children's rheumatology department, one girl was hospitalized in the department for older children in Fakultní Nemocnice Ostrava, clinic of paediatrics, and one boy was hospitalized in hospice centre St. Luke in Ostrava. Information was gathered by means of structured non-standardised interview with the child and a parent and by observation. Secondary source of information was medical documentation and records. With patient?s permission the information was recorded and transcribed to electronic form. The received data was sorted into thematic categories and subcategories according to the psycho-biological and social needs of a child with chronic pain. Results: The research revealed that chronic pain causes serious complications in life for child patients and their parents. The research results showed that what children and their parents need the most is a functional family, good interpersonal relationships, adequate information, real interest of medical staff and a holistic approach to the child patient. Their greatest wish is to recover, not to have chronic pain of joints, and to stop taking and applying medication and cytostatics. The main needs of these patients are the state without chronic pain, health and a full life as it was before the outbreak of chronic disease and pain. Conclusion: In conclusion, long-term chronic pain deeply affects the child. It is often connected with unpleasant experiences of fear, anxiety, and the child suffers on psycho-biological and social level. It is a significant burden on both the patients and their families and has negative effects on their lives, it was most notable on the physical, educational and emotional level.
