Selected types of bacterial resistance and their rapid detection

Abstract:

Úvod: Bakteriální rezistence k antimikrobiálním přípravkům je v současné době neutichající fenomén. Je možné hovořit o stále rostoucím trendu, jak u gramnegativních bakterií s produkcí β-laktamáz typu ESBL nebo karbapenemáz, tak i grampozitivních např. MRSA nebo VRE. Rychlá diagnostika se stanovením citlivosti je klíčová pro všechny pacienty a zvláště ty, kteří jsou v ohrožení života. Práce se zabývá vývojem rychlé metodiky pro detekci vybraných genů pro ESBL, MRSA a VRE přímo z klinického materiálu. Materiál a metody: Z celkového počtu aktuálně popsaných β-laktamáz typu ESBL byla vyvinuta metoda pro detekci genů blaSHV-ESBL a blaCTX-M-ESBL vedoucích k produkci enzymů typu SHV a CTX-M. Dále byla vyvinuta metodika pro detekci MRSA, a to všech dosud popsaných stafylokokových chromozomových kazet (I.-XI.), které jsou zodpovědné za vznik rezistence k methicillinu. Také byla vyvinuta metodika pro detekci vanA a vanB genů, které vedou ke vzniku VRE. U všech byla použita metoda real-time PCR. Detekce byla provedena přímo z izolovaného klinického materiálu bez nutnosti předchozí kultivace. Metodika byla optimalizována na sbírkových kmenech, stanovena cross-reaktivita vůči ostatním patogenům, a nakonec ověřena na souboru klinických vzorků, u kterých byla stanovena klinická senzitivita, specificita, pozitivní (PPV) i negativní (NPV) prediktivní hodnota. Kvalita všech tří metod byla ověřena na panelech externího hodnocení kvality (EHK). Výsledky: Metoda pro detekci genů vedoucích k ESBL byla ověřena na 341 vzorcích endotracheálních aspirátů a na 84 vzorcích klinického materiálu, který pocházel od hematoonkologických pacientů (nejčastěji stěr z krku, stolice, moč). U vzorků endotracheálních aspirátů byla stanovena 96% senzitivita, 89% specificita, PPV = 43 % a NPV = 99,6 %. Metoda pro detekci MRSA detekovala 96 % vzorků klinického materiálu ve shodě s fenotypovými metodami. Metoda pro detekci VRE se 100 % shodovala se zlatým standardem na vzorcích klinického materiálu. Všechny vzorky byly pozitivní na vanA gen. Závěr: Byla vyvinuta metodika pro detekci vybraných genů rezistence přímo z klinického materiálu. Pokud je autorce známo, detekce genů podmiňujících vznik ESBL přímo z klinického materiálu bez předchozí kultivace byla v rámci předkládané práce publikována vůbec jako první. Diagnostika bakteriální rezistence molekulárně-biologickými metodami má významný potenciál využití v klinické praxi.

Abstract:

Introduction: Bacterial resistance to antimicrobials is currently a non-putative phenomenon. It is possible to talk about the ever-increasing trend of gram-negative bacteria with ESBL-type β-lactamase production or carbapenemases as well as gram-positive bacteria such as MRSA or VRE. Rapid diagnostics with the sensitivity determination is crucial for all the patients, especially those who are at the risk of life. This thesis deals with the development of rapid diagnostics for the detection of ESBL, MRSA and VRE directly from the clinical material. Material and methods: The new method for detection of blaSHV-ESBL and blaCTX-M-ESBL genes leading to the production of SHV and CTX-M enzymes was developed from the total number of actually described ESBL β-lactamases. Moreover, the new method for detection of MRSA was developed for all currently described staphylococcal chromosomal cassette (1.-XI.) which are responsible for methicillin resistance. The method for detection vanA and vanB genes which are the cause of VRE was developed. All methods were based on real-time PCR. The detection was performed directly from the extracted DNA of the clinical material without previous cultivation. The methods were optimized on reference strains. The cross-reactivity was tested on the panel of pathogens and finally, the set of clinical samples were used for the assessment of clinical sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV). The validity of all three methods was verified on external quality assessment panels (EQA). Results: The method for detection of ESBL genes was tested on 341 samples of endotracheal aspirate and on 84 samples of other types of clinical material which originated from hematooncological patients (the most common were throat swab, stool and urine). The sensitivity 96 %, specificity 89 %, PPV = 43 % and NPV = 99.6 % was determined with the endotracheal aspirates. The method for detection of MRSA detected 96 % samples of the clinical material with a concordance of phenotypic methods. The method for detection of VRE was fully concordant (100 %) with the gold standard determined on the clinical material samples. All samples were positive to vanA gene. Summary: The new methods for detection of chosen resistance genes directly from clinical material were developed. As far as the author knows, the detection of ESBL genes directly from clinical material without previous cultivation was in this thesis published for the first time. The diagnostics of bacterial resistance by molecular-biology methods have an important potential for clinical practice.
