Nursing care plan for pacient after heart operation - aortic dissection

Abstract:

Bakalářská práce pojednává o stanovení ošetřovatelského plánu pro pacienta, který je po akutní aortální disekci. Disekce je akutní onemocnění aorty, které pacienta ohrožuje na životě. Toto onemocnění se nevyskytuje příliš často ale průběh takovýchto případů je velmi rychlý a vyžaduje urgentní řešení, často operační. Práce je rozdělena na dvě části, teoretickou a praktickou. Teoretická část práce se z počátku zabývá základní anatomií aorty a příčinami onemocnění. Dále je věnována základnímu rozdělení aortálních disekcí, diagnostice a je nastíněna léčba tohoto onemocnění. Poslední kapitola teoretické části se zabývá klinickým obrazem onemocnění a úkoly sester intenzivní péče při monitorování pacienta po příjezdu z operačního sálu. Cílem praktické části práce je popsat zdravotní stav pacienta po operaci na jednotce intenzivní péče. Jsou rozebrány potřeby pacienta po kardiochirurgické operaci na jednotce intenzivní péče a následně je stanoven ošetřovatelský plán péče pro pacienta. U pacienta jsou stanoveny ošetřovatelské diagnózy dle NANDA diagnostiky a k nim přiřazeny ošetřovatelské cíle a intervence, které zdravotní sestra bude u pacienta provádět.

Abstract:

The focus of this bachelor's thesis is on the process of defining the nursing plan for a patient who underwent acute aortic dissection. This is an urgent disease of aorta, which is threatening to patient's life. Such illness does not occur often, but its progress is very fast and requires urgent action, in most cases surgical. The thesis is divided into two parts, theoretical and practical. The theoretical part deals with the fundamental anatomy of aorta and with the cause of this disease. Apart from that, there are described basic division of aorta's dissections, diagnostics and partly the cure. Last chapter of the theoretical part is focused on the clinical picture of the illness and on the work of nurses in the intensive care unit during patient's monitoring after his arrival from the operating room. The practical part deals with patient's health after his operation. Patient's needs in the intensive care unit after a cardiac surgery are discussed in this part of the thesis as well as the nursing care plan for him. Nursing diagnoses are established according to the NANDA diagnostics and specific aims and interventions done by the nurse are also defined there.
