The morphological and functional evaluation of the left ventricle in asymptomatic female carriers of Duchenne muscular dystrophy gene mutations using non-invasive cardiac imaging

Abstract:

Včasná detekce kardiálního postižení u asymptomatických nosiček genu pro Duchennovou muskulární dystrofii je rozhodující pro lepší prognózu. Cílem této dizertační práce bylo provést morfologické a funkční hodnocení levé komory v uvedené populaci pomocí neinvazíivních zobrazovacích metod. Dizertační práce se skládá ze 6 částí. První 3 části představují Duchennovu muskulární dystrofii a s ní asociovanou kardiomyopatii u postižených chlapců a jejich matek (nosiček toho genu) zaměřenou na zobrazovací techniky. Druhé 3 časti zahrnují vlastní výsledky založené na předchozí hypotéze, že u asymptomatických nosiček tohoto genu se zachovalou ejekční frakcí levé komory se může objevit podobné, i když mírnější kardiální postižení, detekované právě pomocí pokročilých echokardiografických zobrazovacích metod, kardiální magnetické rezonance feature tracking, T1 nativního mappingu a extracelulárního objemu. Věříme, že využití moderních neinvazivních zobrazovacích vyšetrovacích technik během pravidelných vyšetření může přispívat ke včasné diagnostice kardiálního postižení u nosiček genu tohoto onemocnění.

Abstract:

Detection of cardiac involvement in asymptomatic female carriers of Duchenne muscular gene mutations is crucial for a better outcome. The aim of the thesis was to provide a morphological and functional assessment of the left ventricle in this population using non-invasive cardiac imaging. The dissertation thesis consists of 6 parts. The first 3 parts introduce Duchenne muscular dystrophy and the associated cardiomyopathy in affected boys and their mothers (female carriers) focused on imaging techniques. The second 3 parts include our own results - based on the previous hypothesis that cardiac injury similar to that in the affected boys albeit milder, can appear in asymptomatic female carriers of these gene mutations with preserved left ventricular ejection fraction, found using advanced echocardiography methods, cardiac magnetic resonance feature tracking and also T1 native mapping and extracellular volume. We believe that modern non-invasive imaging techniques used in regular examinations can contribute to earlier diagnosis of cardiac damage in female carriers of these gene mutations.
