The practical utilization of vascular access for dialysis

Abstract:

Diplomová práce vychází ze sledování využití jednotlivých typů cévních přístupů v klinické praxi u pacientů léčených hemodialýzou. Data byla získána prospektivně a retrospektivně ze zdrojové dokumentace pacientů léčených v chronickém dialyzačním programu v rámci jednoho dialyzačního střediska. Porovnává zastoupení jednotlivých typů cévních přístupů ve dvou skupinách pacientů s odstupem deseti let a hodnotí je z pohledu mezinárodních guidelines. Dále se práce věnuje analýze faktorů, které ovlivňují použití jednotlivých cévních přístupů v praxi. Bylo zjištěno, že v praxi se daří dodržet požadovanou četnost použití nativní A-V fistule, avšak četnost použití permanentního centrálního venózního katétru je výrazně vyšší než stanovují odborná doporučení. Na základě výsledků výzkumu nebyl identifikován žádný klíčový faktor. Byla navržena možná opatření, která by mohla v praxi přispět ke změně.

Abstract:

The diploma thesis is based on monitoring the use of individual types of vascular access in clinical practice in patients treated with hemodialysis. Data were obtained prospectively and partially retrospectively from source documentation of patients treated in a chronic dialysis program within one dialysis centre. It compares the representation of individual vascular access types in two groups of patients ten years apart and evaluates them from the perspective of international guidelines. Furthermore, the thesis deals with the analysis of factors that affect the use of individual vascular accesses in practice. It was found that it is possible to adhere to the required frequency of use of the native AV fistula in practice, but the frequency of use of the permanent central venous catheter is significantly higher than the professional recommendations. No key factor was identified based on the research results. Possible measures have been suggested that could contribute to change in practice.
