Internal fixation of distal tibial fractures

Abstract:

Cílem práce je srovnání dvou základních druhů vnitřní osteosyntézy, které si v oblasti distálního bérce konkurují, tedy nitrodřeňového zajištěného hřebování a dlahové osteosyntézy, na základě sledování rozdílů v četnosti různých druhů komplikací, délky hojení zlomeniny a funkčních výsledků. Za účelem porovnání byla provedena prospektivní studie, do které byli zahrnuti pacienti, kteří byli léčeni pro extraartikulární zlomeninu distálního bérce v Úrazové nemocnici v Brně v letech 2005-2009. Dlahovou osteosyntézou bylo léčeno 38 pacientů (skupina A), 53 pacientů bylo léčeno nitrodřeňovým zajištěným hřebem (skupina B). Ve skupině A byl vyšší výskyt infekčních komplikací, ve skupině B zase větší zastoupení malpozic a selhání osteosyntézy. Rozdíly u těchto dvou druhů komplikací ale nebyly statisticky významné a v ostatních parametrech byly nepatrné. Oba postupy lze v současnosti považovat za standardní a mají své místo v léčení extraartikulárních zlomenin distálního bérce. Volba metody je dána erudicí a technickým vybavením pracoviště, v konkrétních případech i očekávaným druhem komplikací.

Abstract:

The aim of thesis is comparison of two basic types of internal fixation which compete in the area of the distal tibial metaphysis, i.e. locked intramedullary nailing and plate osteosynthesis, on the basis of monitoring the incidence of various complications, the duration of the fracture healing process and functional results. In order to comparsion, a prospective study was performed involving patients treated for extra-articular distal tibial fractures at the Accident & Emergency Hospital in Brno between 2005 and 2009. 38 patients (Group A) were treated by means of plate osteosynthesis, 53 patients (Group B) were treated using intramedullary nailing. In Group A there was a greater incidence of infectious complications, while in Group B, there was a higher proportion of malunions and osteosynthesis failures. The differences in those two types of complications however were statistically insignificant, and in other parameters negligible. Both techniques may at present be considered standard procedures, with a place in treatment of extra-articular distal tibial fractures.The method selected is determined by competence and available technical equipment, as well as by expected complications in specific cases.
