Occupational Therapy in Patient with Dislocation of the Carp

Abstract:

Úvod: Luxace karpu, neboli vykloubení zápěstí, je úraz, který značnou mírou a v mnoha oblastech negativně zasahuje do života člověka. Jedná se o poměrně raritní záležitost, a proto je dobré o tomto tématu získat větší povědomí. Ergoterapeut má za úkol včas a správně zahájit rehabilitaci a tím minimalizovat možné nežádoucí dopady úrazu. Cíl práce: Cílem bakalářské práce je přispět k řešení problematiky ergoterapeutické intervence u pacienta s luxací karpu. Metodika: V hlavní části je představena kazuistika pacientky po luxaci karpu a u ní uplatněná ergoterapeutická intervence. Na základě zdravotnické dokumentace a semistrukturovaného rozhovoru byla odebrána anamnéza. Vyšetření probíhaly formou aspekce, palpace, testování a měření. Výsledky: Pacientka se po úrazu zotavila a podařilo se zlepšit aktivní pohyby i svalovou sílu, která je však stále subjektivně na nižší úrovni, než před úrazem. Jizva je v dobrém stavu, otok téměř vymizel. Parestezie však přetrvává, nicméně hlavní příčinou je dle EMG (elektromyografie) syndrom karpálního tunelu, který byl nejspíše vlivem úrazu zhoršen. Závěr: Celkově hodnotím terapii jako úspěšnou. Pacientka ruku aktivně zapojuje a je plně soběstačná.

Abstract:

Introduction: A carpal luxation, or wrist dislocation, is an injury that significantly and in many areas negatively affects a person's life. It is a relatively rare occurrence and it is therefore a good idea to raise awareness of the subject. The occupational therapist has the task of initiating rehabilitation in a timely and correct manner, thereby minimising the possible adverse effects of the injury. The aim of the thesis: The aim of the bachelor thesis is to contribute to solving the problem of occupational therapy intervention in a patient with dislocation of the carp. Methods: In the main part, a case report of a patient after carpal luxation and the occupational therapy intervention applied to her is presented. Medical records and semistructured interview were used to take a medical history. Examinations were performed by gaze examination, palpation, testing and measurement. Results: The patient recovered from the injury and managed to improve her active movements and muscle strength, which is still subjectively at a lower level than before the injury. The scar is in good condition, the swelling has almost disappeared. Paresthesia persists, however, the main cause is carpal tunnel syndrome (according to electromyography results), which was probably aggravated by the injury. Conclusion: Overall, I rate the therapy as successful. The patient actively engages her hand and is fully self-sufficient.
