Analysis of the manifestations of intolerance to metal materials depending on the number and type of fixed and removable works in the oral cavity in patients with Tako-tsubo cardiomyopathy, properties of filling materials and the genetic background of recurrent aphthous stomatitis (RAS)

Abstract:

Disertační práce je zaměřena na problematiku vlivu dentálních materiálů na pacienty a fyziologickou reakci v dutině ústní. První část práce se věnuje alergické reakci na kovy u pacientů s Tako-tsubo kardiomyopatií, která byla sledována pomocí MELISA testu. Pacienti vykazovali nejvyšší reaktivitu na nikl (Ni), rtuť (Hg), cín (Sn), stříbro (Ag) a kobalt (Co). Druhá část práce je zaměřena na výplňové materiály, používané v konzervačním zubním lékařství. V in vitro studii bylo analyzováno celkem 10 materiálů (amalgámy, skloionomerní cementy a kompozitní materiály). Byl hodnocen vliv 12 různých dokončovacích a leštících technik na strukturu povrchu materiálů. Pomocí digitální mikroskopie a mikroprofilování byly vypočteny hodnoty povrchové drsnosti (Ra, Rp, Ry) materiálů před a po dokončovací nebo leštící úpravě. Analýza ukázala, že nejnižších hodnot Ra, Rp a Ry bylo dosaženo u materiálu NanoWise a Filtek Flow. Nejhladší povrch byl dosažen diamantovým brouskem se žlutým pruhem, měkkou žlutou gumičkou, žlutým Sof-lex diskem a finýrkou. Třetí část práce se věnuje pacientům s recidivující aftózní stomatitidou (RAS) a roli polymorfismů genu angiotenzin I konvertujícího enzymu při tomto onemocnění dutiny ústní. Bylo zjištěno, že AGTGD a haplotyp může zvyšovat riziko rozvoje RAS, zatímco CGI haplotyp může být vůči RAS považován za ochranný faktor.

Abstract:

The PhD study focuses on the influence of dental materials on patients and the physiological response in the oral cavity. The first part of the thesis is devoted to the allergic reaction to metals in patients with Tako-tsubo cardiomyopathy, which was monitored using the MELISA test. Patients showed the highest reactivity to nickel (Ni), mercury (Hg), tin (Sb), silver (Ag) and cobalt (Co). The second part of the study focuses on filling materials used in restorative dentistry. A total of 10 materials (amalgams, glass ionomer cements and composite materials) were evaluated in an in vitro study. The effect of 12 different finishing and polishing techniques on the surface texture of the materials was evaluated. Using digital microscopy and microprofiling, the surface roughness values (Ra, Rp, Ry) of the materials before and after finishing or polishing were calculated. The analysis showed that the lowest values of Ra, Rp and Ry were achieved for NanoWise and Filtek Flow materials. The smoothest surface was achieved with a diamond bur with yellow label, a soft yellow rubber, a yellow Sof-lex disc and a finisher. The third part of the thesis focuses on patients with recurrent aphthous stomatitis (RAS) and the role of angiotensin I converting enzyme gene polymorphisms in this oral disease. It has been found that AGTGD haplotype may increase the risk of developing RAS, while the CGI haplotype may be considered a protective factor against RAS.
