Reorganizace řečových kortikálních okruhů u pacientů s epilepsií temporálního laloku v obraze funkční magnetické rezonance

Reorganization of language cortical network in patients with temporal lobe epilepsy in wiev of functional magnetic resonance

Abstract:

Funkční magnetická rezonance je moderní zobrazovací metoda sloužící k zobrazování činnosti mozku, resp. mapování cerebrální odezvy na vnější či vnitřní podnět. Rozvíjí se zejména v posledních deseti letech a za tuto dobu do značné míry obohatila poznání v oblasti kognitivních věd a neurofyziologie. Významný přínos z hlediska výzkumu řečového kortexu představuje možnost detailnějšího vhledu do normálního funkčního uspořádání jazykových respektive řečových neuronálních systémů. Pomocí funkční magnetické rezonance můžeme mapovat primární řečové oblasti, ale i další heteromodální kortikální a extrakortikální regiony zapojené do procesů spojených s řečí. Nové metody analýzy funkčních dat jako efektivní konektivita nám navíc umožňují posouzení vzájemných interakcí mezi těmito oblastmi. Cílem této práce je porovnání řečových kortikálních okruhů u zdravých subjektů a pacientů s levostrannou temporální epilepsií. Vliv epileptiformní aktivity na organizaci řečového kortexu u pacientů je zmiňován již v předchozích studiích založených na Wada testu (intrakarotický barbiturátový test sloužící k testování jazykových a paměťových funkcí před plánovaným epileptochirurgickým zákrokem). Vyšší procento výskytu atypické (pravostranné či smíšené) řečové dominance u pacientů s levostrannou temporální epilepsií předpokládá potenciální interhemisferický přesun řečových funkcí. Funkční magnetická rezonance umožňuje ve srovnání s Wada testem mnohem podrobnější analýzu konsekvencí mezi epilepsií a předpokládanou reorganizací řečového kortexu. Kromě interhemisferického přesunu dokáže rovněž posoudit reorganizaci v rámci řečově dominantní hemisféry, tedy intrahemisfericky. Mozek epileptických pacientů tak může sloužit jako progresivní model pro studium plasticity cerebrálních funkcí. V souhrnu, z našich výsledků je zřejmé, že funkční organizace neuronální sítě pro řeč je u pacientů s levostrannou temporální epilepsií určitým způsobem modifikována. Pro potenciální reorganizaci svědčí kromě výsledků konvenční funkční magnetické rezonance i porovnání lateralizačních indexů (kvantifikace popisných výsledků) a skupinové výsledky analýzy efektivní konektivity (modelování interakcí mezi řečovými oblastmi). Ne všechny výsledky této práce dokážeme spolehlivě interpretovat, přesto jsme již schopni poměrně bezpečně určit typický levostranný „řečový vzorec“. Část pacientů s farmakorezistentní epilepsií temporálního laloku by tak již nemusela v rámci předoperační přípravy absolvovat invazivní Wada test, ale pouze rutinní fMRI vyšetření.

Abstract:

Functional magnetic resonance imaging is a modern neuroimaging method that serves to display the functions of the brain by mapping cerebral response to internal and/or external stimuli. It has developed significantly over the last ten years; during this time it has increased essential knowledge in the realms of cognitive science and neurophysiology. It has enabled a more detailed insight into the normal functional organisation of neuronal system associated with speech. This has provided a meaningful contribution to language cortex research. With the help of functional magnetic resonance imaging, it is possible to map the primary speech areas as well as other heteromodal cortical and extracortical regions involved in speech processes. New methods of functional data analysis, such as effective connectivity, additionally enable the evaluation of interaction between these areas. The aim of this work is to compare the cortical networks related to speech in healthy subjects and patients suffering left temporal lobe epilepsy. The impact of epileptiform activity on the organization of these cortical areas in patients has already been mentioned in studies based on the Wada test (intracarotid barbiturate procedure serving to assess language and memory function prior to the surgical treatment of the seizures). With the higher incidence of atypical (right or mixed) hemispheric language dominance in left temporal epilepsy patients, a potential interhemispheric shift of speech function is to be expected. Compared with the Wada test, functional magnetic resonance imaging enables a much more detailed analysis of the connections between epilepsy and the supposed reorganization of the speech cortex. In addition to assessing the interhemispheric shift, functional magnetic resonance imaging can also assess reorganization within the language dominant hemisphere, in other words: intrahemispheric reorganization. Thus, the brains of epileptic patients can serve as a progressive model for studying the plasticity of cerebral functions. In summary, our results clearly indicate that the functional organisation of the language-related neuronal networks in patients suffering from left temporal lobe epilepsy is by some means altered. In addition to the results of conventional functional magnetic resonance imaging, the possible reorganisation is also reflected in a comparison of laterality indexes (quantification of descriptive results) and group analysis of effective connectivity (modelling of interactions between speech areas). We cannot reliably interpret all the results in this study. Nevertheless, we are able to relatively safely identify the typical “speech pattern” for patients with left temporal epilepsy. Some patients suffering pharmacoresistant temporal lobe epilepsy thus need not undergo the invasive. Wada test usually associated with preoperative investigation, but instead can be diagnosed with a routine functional magnetic resonance imaging examination.
