Abstract:

Úvod: Parametrizace informačních systémů je významným úkolem současné zdravotnické informatiky, neboť bez ní není možné rozvíjet systémy elektronického zdravotnictví, hodnotit kvalitu a výsledky péče a v neposlední řadě zajišťovat odpovídající zázemí výzkumu a klinickým studiím. Celosvětově můžeme sledovat velmi výrazný trend v oblasti provádění klinických zkoušení, který je významným stimulem i pro oblast zdravotnické informatiky a statistiky. Současný nástup personalizované medicíny a rostoucí heterogenita dostupných diagnostických a léčebných postupů vyžadují adekvátní vývoj i na straně struktury a funkcionality informačních systémů. Na druhé straně je třeba reflektovat rostoucí etické nároky a legislativní tlak zejména v oblasti ochrany osobních údajů, který má dokonce potenciál zabrzdit dosud výrazně rostoucí objem klinického výzkumu. Oficiální zdroje National Institute of Health (www.clinicaltrials.gov) v USA uvádějí, že za posledních 14 let bylo provedeno více jak 175 tis. klinických studií. Každoroční přírůstek počtu nových klinických hodnocení je přibližně 20 % oproti předchozímu roku. Pokud přijmeme předpoklad, že oficiální klinické studie tvoří 10 % podílu na celkových studiích vč. neintervenčních, post-marketingových a studiích bezpečnosti, dostáváme se k velmi vysokému počtu realizovaných zkoušení. Pro rok 2014 tedy můžeme odhadovat, že celosvětově probíhá více jak 140 tis. klinických studií. Problematika klinického zkoušení klade velký důraz na bezpečnost a soukromí pacienta, což musí v první řadě řešit zapojené informační systémy a jejich aplikace. Hlavním cílem této práce bylo souhrnně popsat tuto problematiku, navrhnout a implementovat specifické řešení pro informační a databázové zázemí klinických studií i běžné klinické praxe, které bude splňovat náročná legislativní kritéria pro bezpečnost pacienta, ale zároveň nebude limitovat vlastní účel sběru dat. Metody: Na základě zkušeností s projektovým managementem v oblasti neintervenčních studií a klinických registrů jsem se zaměřil především na situaci v EU, konkrétně v ČR, kde dosud neexistuje jednotný a komplexní názor na problematiku anonymizace klinických dat. Na základě rešerše jednotlivých zákonů zabývajících se ochranou osobních údajů a zpracování dat v oblasti klinického zkoušení jsem vyvinul metodiku sběru dat pomocí autonomní deanonymizované struktury, která byla následně implementována v konkrétních klinických projektech a studiích, včetně celorepublikově probíhajících hodnocení inovativní léčby. Hodnocení léčby v klinické praxi má v současnosti kvalitní a mezinárodně akceptovaný metodický základ sledující obecné principy hodnocení zdravotnických technologií (“Hospital-based HTA”), kterého jsme se drželi. Výsledky: Výsledkem této práce je oficiálně publikovaná metodika, která byla implementována do podoby softwarového řešení a již přinesla konkrétní výsledky z klinické praxe. Celá koncepce byla následně použita na praktickém případu projektu zavedení evidence a hodnocení vysoce inovativních léčebných přípravků (VILP) v ČR, dle platné legislativy. Celostátně uznaný a využívaný systém sběru a hodnocení klinických dat je hlavním metodickým výstupem disertační práce. Jeho modulární a flexibilní struktura je zárukou udržitelnosti a rostoucího počtu aplikací. Aplikovaná metodika respektuje logickou strukturu a časovou posloupnost diagnostiko-léčebných algoritmů a nijak nelimituje výzkumníka či lékaře co do obsahu sbíraných dat. tí medicíny při respektování jejich specifických požadavků, např. na parametrizaci výstupů léčby. Důkazem jsou mezinárodní publikace, které tvoří část doložených výsledků disertace. Závěr: Řešení prezentované v předložené práci významným způsobem přispělo k realizaci četných neintervenčních multicentrických studií, sběru a analýze dat a publikaci získaných poznatků z klinické praxe. Výstup disertační práce umožnil realizovat hodnocení vysoce inovativní léčby vČR dle novelizovaných legislativních předpisů. Metodika současně vyřešila požadavky kladené na bezpečnost klinických dat a ochranu osobních údajů. Disertační práce prokázala, že vývoj na poli zdravotnické informatiky dnes nelze oddělovat od vývoje klinického a obě oblasti se vzájemně synergicky doplňují.

Abstract:

Introduction: Parameterisation of information systems belongs to important issues of health informatics; it is essential for the development of eHealth systems, assessment of health care results and quality, and, last but not least, ensuring background for research and clinical trials. A significant trend in clinical testing has been observed globally, which also stimulates progress in health informatics and statistics. Recent progress in personalised medicine and increasing heterogeneity in available diagnostic and therapeutic procedures require an adequate development in the information systems structure and functionality. On the other hand, it is necessary to reflect increasing requirements in the field of ethics and legislation, particularly in the personal data protection, which may even slow down the growing volume of clinical research. According to the official information by the National Institute of Health (www.clinicaltrials.gov) state that over 175,000 of clinical trials have been carried out during last 14 years. Annual increment in the number of new trials is approximately 20%. If we accept an assumption that official clinical trials contribute 10% to the total number of trials including non-interventional, post-marketing, and safety studies, we obtain really high number of realised clinical trials. We can therefore estimate the number of trials realised in 2014 around 140,000. Patient’s safety and privacy are among the most important issues in clinical testing, which should be reflected firs of all by employed information systems and their applications. Main objective of this work is describe this field, design and implement a specific solution for information and database background of clinical trials and routine clinical practice, which would meet demanding legislation criteria for patients safety, but would not limit the purpose of data collection at the same time. Methods: Based on the experience with project management in the field on non- interventional studies and clinical registries I focused mainly of the situation in the European Union and specifically in the Czech Republic, where no single and cohesive opinion exists on the clinical data de-identification. I performed a review of individual acts dealing with personal data protection and data processing in the field of clinical research and designed a methodology of data collection using and autonomous structure, which was subsequently implemented in particular clinical projects and trials, including national evaluation of innovative therapies. Internationally accepted principles of health care assessment in clinical practice are provided by so called “Hospital-based HTA”, which we followed in this thesis. Results: Main out of the thesis is an officially published methodology, which was implemented into the software solution and has already brought particular results from clinical practice. The whole conception was subsequently in the project of implementation of highly innovative drugs registering and assessment in the Czech Republic according to the valid legislation. The nationally accepted and employed system of clinical data collection and assessment is a main methodological output of the thesis. Its modular and flexible structure guarantees sustainability and growing number of applications. The applied methodology respects logical structure and time sequence of diagnostic and treatment algorithms and the data content does not limit the researcher or physician in the practice. A universal data model can be filled with specific parameters from various fields of medicine while respecting their specific requirements, e.g., in treatment outputs parameterisation. This is documented by international publications presented in this thesis. ractice. The output of the thesis facilitated assessment of the highly innovative drugs in the Czech Republic according to the amended legislation. The methodology also solved the problems related to the clinical data security and personal data protection. The thesis also documents that progress in the field of health informatics cannot be separated from the clinical progress and both field complement each other synergically.
