Distribution of the Laboratory Screening in First Trimester of Gravidity - Utilize of Examination of Free Beta-Unit hCG in the Diagnostics of DS - Diagnostics of Disease of Thyroid Gland.

Abstract:

Úvod magisterské práce je zaměřen na problematiku prenatální diagnostiky Downova syndromu v I. trimestru gravidity a na imunoanalytické metody stanovení biochemických markerů používaných v rámci provádění screeningu DS. Důraz je kladen především na další biochemický marker - volnou ?-podjednotku hCG v I. trimestru gravidity. Předmětem praktické části magisterské práce je verifikace metody stanovení free ?-hCG realizované na analyzátoru KRYPTOR firmy B.R.A.H.M.S. Součástí verifikace metody je posouzení míry její přesnosti (pomocí sledování opakovatelnosti, reprodukovatelnosti), správnosti metody (porovnáním shody deklarovaných a skutečně nalezených hodnot koncentrací kontrolních vzorků) a robustnosti metody s ohledem na její možnou teplotní závislost, která by mohla hrát určitou roli při výsledném hodnocení rizika DS. Byly zjištěny velmi příznivé parametry přesnosti (výsledky získané sledováním opakovatelností i reprodukovatelností) metody. Sledováním teplotní závislostí metody a podmínek skladování vzorků bylo zjištěny, že vliv podmínek separace a skladování sice může mít vliv na stanovení koncentrace free ? hCG, ale k zásadnímu ovlivnění hodnoty spočítaného rizika DS nedochází. Při posouzení správnosti metody bylo zjištěno, že skutečně naměřené hodnoty v laboratoři vycházejí mírně nižší než odpovídající hodnoty deklarované pro tuto metodu. Druhá část magisterské práce je zaměřena na problematiku tyreopatií u těhotných žen a jejich možného vlivu na plod. Problematika byla posuzována zejména s ohledem na možnosti jejich současné diagnostiky v době provádění sreeningu Downova syndromu během I. trimestru gravidity. V praktické části bylo provedeno stanovení protilátek anti-TPO a hormonu TSH v souboru 164 vzorků sér dodaných do laboratoře za účelem provedení screeningu DS. Bylo hodnoceno, zda v populaci žen vyšetřovaných v IAL KNM během těhotenství existují patologické nálezy těchto markerů, svědčící o riziku poruchy funkce štítné žlázy. Po zpracování naměřených hodnot bylo zjištěno, že u významného podílu těhotných žen byly nalezeny patologické hodnoty anti-TPO nebo hormonu TSH. Z tohoto důvodu by tak screeningové vyhledávání tyreopatií u těhotných bylo účelné a mohlo by se realizovat v I. trimestru gravidity.

Abstract:

The introduction of the master's thesis is focused on problems of prenatal diagnostics of Down's syndrome in the I. trimester of gravidity and on immuno analytical methods of assessment of biochemical markers used as part of DS screening. Stress was put especially on the biochemical marker free ?-hCG in the I. trimester of gravidity. The subject of the practical part of the master's thesis is a verification of the method of assessment of free ?-hCG, carried out on KRYPTOR analyser, from B.R.A.H.M.S. company. A part of the method verification is also evaluation of its degree of accuracy (through the use of evaluation of repeatability, reproducibility), correctness of the method (by comparison of conformity of declared and actually found values of concentration of control samples) and robustness of the method with regard to its possible temperature dependence that could play a role during final evaluation of DS risk. Very favourable values of both repeatability and reproducibility of the method were found by accuracy of the method. By temperature dependence and storing of samples, it was found out that conditions of separation and storing do not significantly influence assessment of free ? hCG through this method. By correctness of the method, it was found out that actually measured values are lower than the values declared by this method. The second part of the master?s thesis is focused on problems of thyreopatia of gravid women and its potential effect on fetus. The problems were evaluated particularly with regard to possibility of its simultaneous diagnostics at the time when screening of Down?s syndrome in the I. trimester is carried out. In the practical part, an assessment of antibodies anti-TPO and TSH hormone was carried out on set of 164 samples of serums delivered to laboratory for the purpose of DS screening. It was evaluated whether in population of women examined in IAL KNM during gravidity there were pathologically increased values of these markers indicating a risk of thyroidea functional disease. After the analyses of measured values, it was found out that some gravid women had pathologically increased values of anti-TPO and TSH hormone. That is the reason, why also screening examination of thyroidea should be carried out in the I. trimester of gravidity.
