Brachycephalic obstructive airway syndrome in dogs

Abstract:

Psí brachycefalie je charakterizována krátkou a širokou lebkou s různě zkráceným čenichem. Dolní čelist zůstává obvykle relativně normálně dlouhá, kdežto horní čelist je výrazně zkrácena. To má za následek předkus, který je pro tyto psy také typický. Brachycefalický tvar lebky je dědičným znakem, který je kontrolován několika geny. Mezi kandidátní geny se řadí FGF4, SMOC2, THBS2, BMP3 a P2RX7. Nejběžnějšími brachycefalickými plemeny jsou francouzský buldoček, anglický buldok, mops, bostonský teriér, německý boxer a pekingský palácový psík. Následkem tohoto extrémního fenotypu se u některých z těchto psů může vyvinout brachycefalický syndrom. Brachycefalický syndrom je charakterizován kombinací primárních a sekundárních abnormalit, které vedou k částečné nebo úplné obstrukci horních cest dýchacích. Nejčastěji postižená plemena jsou francouzský buldoček, anglický buldok a mops. Vyšší riziko vzniku BS mají obézní psi a psi s extrémnějšími brachycefalickými rysy. Primární anatomické abnormality během inspirace způsobují odpor a zvyšují negativní tlak v rámci horních cest dýchacích. Tento negativní tlak může vyvolat sekundární patologické změny, jako je například zesílení měkkého patra, everze laryngeálních váčků, laryngeální kolaps nebo jiné abnormality, které způsobují další obstrukci v již zúženém lumen. Nejběžnější abnormality zahrnují stenotické nozdry, aberantní turbinálie, prodloužené a zesílené měkké patro, relativní makroglosii, evertované laryngeální váčky, kolaps hrtanu, hypoplastickou tracheu a abnormality průdušek. Kvůli těmto anatomickým abnormalitám vznikají u psů různé klinické příznaky. Mezi nejběžnější symptomy BS patří inspirační dyspnoe, stridor, stertor, intolerance pohybu, problémy s termoregulací nebo poruchy spánku. Postižení psi mohou mít také gastrointestinální příznaky, jako je například regurgitace, zvracení, hiatální hernia nebo gastroezofageální reflux. Diagnostika je obvykle založena na přítomnosti klinických příznaků a fyzickém vyšetření. Časná diagnostika a chirurgické řešení BS jsou důležité, protože se zvyšuje šance, že zlepšení proudění vzduchu může minimalizovat progresi sekundárních změn. Chirurgické korekce mohou být pro tato plemena potenciálně nebezpečné, protože mají vyšší riziko vzniku pooperačních komplikací. Existuje mnoho různých operací, které redukují obstrukce dýchacích cest a klinické příznaky u postižených psů. Například rinoplastika, stafylektomie, folded flap palatoplasty, arytenoidní lateralizace, resekce evertovaných váčků, laser asistovaná turbinektomie (LATE) nebo laser asistovaná turbinektomie kaudálních aberantních turbinálií (CAT LATE). U většiny psů dochází po chirurgické korekci ke zlepšení klinických příznaků. Pokud ke zlepšení nedojde, může být jediným možným řešením permanentní tracheostomie nebo eutanazie.

Abstract:

Canine brachycephaly is characterized by a short and wide skull with a variously shortened muzzle. The lower jaw usually remains relatively normally long, while the upper jaw is significantly shortened. This results in an underbite, which is also typical for these dogs. The brachycephalic shape of the head is an inherited trait that is controlled by several genes. The candidate genes include FGF4, SMOC2, THBS2, BMP3, and P2RX7. The most common brachycephalic breeds are French bulldog, English bulldog, Pug, Boston terrier, German boxer, or Pekingese. Due to this extreme phenotype, some of these dogs may develop brachycephalic obstructive airway syndrome. Brachycephalic obstructive airway syndrome is characterized by a combination of primary and secondary abnormalities that lead to partial or complete obstruction of the upper airways. The most affected breeds are French bulldog, English bulldog, and pug. Obese dogs and dogs with more extreme brachycephalic features are at higher risk of developing BOAS. Primary anatomical abnormalities during inspiration cause airway resistance and increase negative pressure within the upper respiratory tract. This negative pressure can cause secondary pathological changes, such as thickening of the soft palate, everted laryngeal saccules, laryngeal collapse, or other abnormalities that cause further obstruction in the already narrowed lumen. The most common abnormalities include stenotic nares, aberrant turbinates, elongated and thickened soft palate, relative macroglossia, everted laryngeal saccules, laryngeal collapse, hypoplastic trachea, and bronchial abnormalities. Due to these anatomic abnormalities, dogs experience various clinical signs. The most common clinical signs of BOAS consist of inspiratory dyspnea, stridor, stertor, exercise intolerance, thermoregulation problems, or sleep disorders. Affected dogs can also have gastrointestinal symptoms such as regurgitation, vomiting, hiatal hernia, or gastroesophageal reflux. Diagnosis is usually based on the presence of clinical signs and physical examination. Early diagnosis and surgical intervention of BS are important because there is a higher chance that improving airflow could minimalize the progression of secondary changes. Surgical corrections can be potentially dangerous for these breeds because they have a higher risk of developing postoperative complications. Many different surgeries reduce airway obstruction and clinical signs in affected dogs. For example, rhinoplasty, staphylectomy, folded flap palatoplasty, arytenoid lateralization, laser-assisted turbinectomy (LATE), and laser-assisted turbinectomy of caudal aberrant turbinates (CAT LATE). In most dogs, clinical signs improve after surgical correction. If there is no improvement, a permanent tracheostomy or euthanasia may be the only solution.
