Rehabilitation for painful shoulder syndrome

Abstract:

Cíl: Cílem bakalářské práce je přispět k řešení fyzioterapie u syndromu bolestivého ramene, konkrétně s diagnózou impingement syndormu a shrnout fyzioterapeutické postupy, koncepty a metody pro terapii bolestivých ramen. Dále seznámit veřejnost s teoretickými i praktickými poznatky, které se týkají této problematiky. Metodika: V hlavní části jsem zpracovala kazuistiku muže, kterému byl diagnostikován impingement syndrom pravého ramene. Na základě vstupního kineziologického rozboru jsem sestavila krátkodobý fyzioterapeutický plán. Následně jsem uskutečnila terapii, která se skládala celkem z dvanácti intervencí. Využila jsem především konceptu proprioceptivní neuromuskulární facilitace, Brügger koncept, SM systém na zlepšení celkového držení těla, techniku měkkých tkání (TMT), mobilizační techniky a postizometrickou relaxaci. Pacientovi bylo zadáno cvičení, které měl vykonávat sám. Před zahájením sedmé intervence jsem odebrala kontrolní kineziologický rozbor a 1.3.2020 jsem odebrala kineziologický výstupní rozbor. Na základě výstupního rozboru jsem sestavila dlouhodobý fyzioterapeutický plán. Výsledky: Již v kontrolním kineziologickém rozboru je viditelné zlepšení pacientových výsledků. Došlo ke zmírnění bolesti v pravém ramenním kloubu a ke zlepšení rozsahu pohybu. Dále došlo ke zlepšení celkového držení těla, zlepšení posturálních funkcí, zmírnění palpační citlivosti v oblasti AC, SC skloubení a 1. a 2. žebra. Závěr: Vybraný fyzioterapeutický postup se prokázal jako prospěšný pro léčbu bolestivého ramene.

Abstract:

Objective: The aim of this assignment is to inform about the problem on shoulder pain, specifically in impingement syndrome and to put together different physiotherapeutic procedures, concepts and methods that are suitable for it's therapy. I would like to acquaint the public with theoretical and practical knowledge of impingement syndrome and to the contribution the of solving this problem. Methodology: In the main part of this thesis I arranged a casuistry of a man who was diagnosed with impingement syndrome of right shoulder. Based on input of the kinesiologal analysis I compiled short-term physiotherapeutic plan. Then I performed a therapy, which was consisted of total 12 interventions. I mainly used the concept of proprioceptive neuromuscular facilitation, the Brügger concept, the SM system for improving the overall posture, the soft tissue technique (TMT), mobilization techniques and post-isometric relaxation. Before starting of the 7th intervention I did a control kinesiological analysis and on 1. 3. 2020 I did an output kinesiological analysis. Based on the output analysis I compiled a long-term physiotherapeutic plan. Results: Patient's condition has already improved during the therapy. The pain in his right shoulder joint was relieved and the range of motion was also improved. Furthermore, the overall posture improved, postural functions improved, palpation sensitivity in the area AC and SC in first and second rib was reduced. Conclusion: Selected physiotherapeutic procedure has proven to be successful for shoulder pain treatment.
